تاثیر زمان تزریق اکسی توسین عضلانی بر طول مرحله سوم زایمان

پیش زمینه و هدف: خطر خونریزی بعد از زایمان با طولانی شدن مرحله سوم زایمان بیش از 30 دقیقه، تا 6 برابر افزایش می یابد و اداره فعال مرحله سوم زایمان خطر خونریزی پست پارتوم را تقریباً تا 60 درصد کاهش می دهد. زمان تجویز یوتروتونیک پروفیلاکسی در اداره فعال مرحله سوم در موسسات مختلف متفاوت است. لذا پژوهشی با هدف تعیین تاثیر زمان تزریق اکسی توسین عضلانی بر طول مرحله سوم زایمان انجام گردید.مواد و روش کار: پژوهش حاضر کارآزمایی بالینی بر روی 100 زن باردار با حاملگی 4238 هفته، تک قلو که به روش نمونه گیری در دسترس انتخاب شدند و به طور تصادفی در دو گروه مداخله ( تزریق 10 واحد اکسی توسین عضلانی بعد از خروج شانه قدامی ) و کنترل ( تزریق 10 واحد اکسی توسین عضلانی بعد از خروج جفت )تخصیص یافتند.ابزار گردآوری اطلاعات شامل پرسشنامه، چک لیست و کرنومتر بود. پس از جمع آوری اطلاعات، مورد تجزیه و تحلیل آماری قرار گرفت.یافته ها: یافته های پژوهش نشان داد که میانگین طول مرحله سوم زایمان در گروه مداخله 7.92 دقیقه و در گروه کنترل 8.86 دقیقه است. اختلاف آماری معنی داری مشاهده نشد. 0.31

THE EFFECT OF INTRAMUSCULAR INJECTION OF OXYTOCIN ON THE DURATION OF THIRD STAGE LABOR

Received: 30 Nov , 2013 Accepted: 1 Feb , 2014AbstractBackground Aims: Prolonged third stage of labor longer than 30 minutes, increase up to sixfold risk of postpartum hemorrhage .While active management of the third stage decrease 60% risk of postpartum hemorrhage. However, different organizations recommended different times for prescribing prophylaxis oxytocin injection.This Study aimed to determine the effect of intramuscular injection of oxytocin on the duration of third stage labor. Materials Methods: A quasiexperimental study conducted on 100 pregnant women with gestational age 4238 weeks, singleton pregnancy. Subjects randomly divided into two groups of injection oxytocin IM with 10 IU after delivery of the anterior shoulder and receiving standard care (injection oxytocin after delivery of the placenta) following a coin toss. A questionnaire, checklist and chronometer were used to collect information . The data were analyzed using SPSS11,5.independent t test were used to analyze data. by using SPSS 11.5 software.Results: The results showed that duration of the third stage in intervention group was 7,92plusmn 2,5.. minutes versus 8,86plusmn 3,1 in the control group. Therefore, the difference of the duration of the third stage of labor was not statistically significant between the two groups (P> 0/31)Conclusion: Injection time of oxytocin IM does not have any effect on the duration of the third stage.
