بررسی تاثیر برنامه مراقبت معنوی بر تبعیت از درمان بیماران تحت همودیالیز

پیش‌زمینه و هدف: عدم تبعیت از درمان در بیماری‌های مزمن و مخصوصاً در بیماران تحت همودیالیز هنوز به‌عنوان یک چالش مطرح است که نیازمند استفاده از مداخلات پرستاری اثربخش است. این پژوهش باهدف تعیین تاثیر برنامه مراقبت معنوی بر تبعیت از درمان بیماران تحت همودیالیز انجام شد.مولد و روش کار: مطالعه حاضر یک مطالعه کارآزمایی بالینی تصادفی شده دو گروه با طرح پیش‌آزمون- پس‌آزمون است که بر روی بیماران تحت همودیالیز مراجعه‌کننده به بخش‌های همودیالیز بیمارستان‌های شهر ارومیه در نیمه دوم سال 1402 انجام شد. در گروه‌های مداخله و کنترل مراقبت‌های روتین بخش ادامه داشت. برای بیماران گروه مداخله، برنامه مراقبت معنوی شامل چهار جلسه یک‌ساعتی بود که به‌صورت انفرادی و دو بار در هفته بعد از انجام همودیالیز در یک اتاق مناسب در بخش همودیالیز انجام شد. جهت جمع‌آوری داده‌ها از پرسشنامه‌های اطلاعات دموگرافیک بیماران و تبعیت از درمان در بیماران مبتلا به نارسایی مزمن کلیه کیم و همکاران (2010) قبل و بعد از مداخله استفاده شد. داده‌ها با نرم‌افزار آماری spss نسخه 22 و با استفاده از آزمون‌های آماری توصیفی و استنباطی تحلیل شدند.یافته‌ها: دو گروه ازنظر مشخصات جمعیت شناختی تفاوت آماری معنی‌داری نداشتند و همگن بودند. میانگین نمرات تبعیت از درمان بیماران قبل از مداخله در گروه آزمون 85.62 ± 678.37 و در گروه کنترل 107.62 ± 682.69 بود که تفاوت آماری معنی‌داری نداشتند (0.848=p). بعد از مداخله در گروه آزمون به 139.98 ± 897.97 و در گروه کنترل به 104.21 ± 694.87 رسید که تفاوت بین آن‌ها معنی‌دار بود (0.001>p) و در گروه آزمون ارتقا پیدا کرده بود.بحث و نتیجه‌گیری: یافته‌ها نشان داد که برنامه مراقبت معنوی در ارتقای تبعیت از درمان بیماران تحت همودیالیز موثر است لذا پیشنهاد می‌شود که پرستاران شاغل در بخش‌های همودیالیز در کنار مداخلات پرستاری به بعد معنوی بیماران توجه نمایند و برنامه مراقبت معنوی را در برنامه‌های خود برای بیماران بگنجانند. 

the effect of spiritual care program on treatment adherence of patients undergoing hemodialysis

background: non-adherence to treatment in chronic diseases, especially in hemodialysis patients, is still a challenge that requires effective nursing interventions. this study was conducted to determine the effect of the spiritual care program on the adherence to treatment of hemodialysis patients.methods: this study is a two-group randomized clinical trial study with a pre-test-post-test design conducted on hemodialysis patients referred to the hemodialysis departments of urmia city hospitals in the second half of 2023. the study comprises two groups: a control group and an intervention group, each receiving routine care. for patients in the intervention group, the spiritual care program consisted of four one-hour sessions conducted individually and twice a week after hemodialysis in a suitable room in the hemodialysis department. in order to collect data, the questionnaires of patients’ demographic information and the end-stage renal disease adherence questionnaire (esrd-aq) (kim et al., 2010) were used before and after the intervention. data were analyzed using descriptive and inferential statistical tests with spss version 22 statistical software.results: the two groups were homogeneous and not statistically significantly different regarding demographic characteristics. the mean score of patients’ adherences to treatment was 678.37 ± 85.62 before the intervention in the intervention group, and it was 682.69 ± 107.62 in the control group, which had no statistically significant difference (p=0.848). after the intervention, it reached 897.97 ± 139.98 in the intervention group and 694.87 ± 104.21 in the control group. the difference between the two groups was significant (p<0.001) and improved in the intervention group.conclusions: the study’s findings demonstrate the effectiveness of the spiritual care program in improving adherence to treatment among hemodialysis patients. this underscores the recommendation that nurses in hemodialysis wards focus on nursing interventions and incorporate the spiritual care program into their patient care plans.
