بررسی چالش‌های استقرار نسل سوم استانداردهای اعتبار بخشی ملی ایران در بیمارستان‌های تابعه دانشگاه آزاد اسلامی شهر تهران

سابقه و هدف: خدمات بیمارستانی با توجه به پیچیدگی آنها نیازمند سیستمی هدفمند برای برنامه ریزی، بهبود و ارزشیابی کیفیت خدمات ارائه شده است. اعتباربخشی، ارزیابی سیستماتیک مراکز ارائه خدمات سلامت با استانداردهای مشخص است که بر بهبود مداوم کیفیت، بیمارمحوری و بهبود امنیت بیمار و کارکنان تاکید دارد. پژوهش حاضر با هدف بررسی چالش های استقرار نسل سوم استانداردهای اعتباربخشی ملی ایران در بیمارستان ها ی تابعه دانشگاه آزاد اسلامی شهرتهران انجام شد.روش بررسی: این پژوهش کاربردی - توصیفی، به صورت مقطعی در بیمارستان بوعلی و امیرالمومنین(ع) انجام گرفت. جامعه آماری، مدیران، کادر پرستاری و پزشکان بودند که از بین آنها، 239 نفرنمونه انتخاب شدند. روش نمونه گیری کادر پرستاری و پزشکان به صورت تصادفی ساده و در مدیران، سرشماری بود. ابزار پژوهش، پرسش نامه ای بود که با معیارپنج گزینه ای لایکرت ]کاملا موافق (5) تا کاملاً مخالف (1)[ تنظیم شده بود. برای تحلیل داده ها از آمار توصیفی و استنباطی anova و نرم افزار20 spss استفاده شد. یافته ها: بیشترین چالش ها، کمبود نیروی انسانی(87.87 %)، زیاد بودن تعداد استانداردها و سنجه ها(81.17 %)، مواجه بودن کادر درمان با شیفت های سنگین کاری(79.92 %)، کمبود بودجه ( 75.73 %) و مشارکت نداشتن پزشکان(62.76 %) بود. نتیجه گیری: نتایج نشان داد بیشترین چالش در امر استقرار استانداردهای اعتباربخشی از دیدگاه مدیران و پزشکان کمبود نیروی انسانی و از دیدگاه کادرپرستاری، مواجه بودن کادر درمان با شیفت های سنگین کاری بودند.

Challenges of implementation of Iranian national hospital accreditation standards (3rd Edition 2016) for Tehran’s Islamic Azad University Hospitals

Background: Hospital services need a targeted system for planning, improving and evaluating the quality of the services provided according to their complexity. Accreditation is a systematic assessment of healthcare centers with specific standards that emphasizes on continuous improvement of quality, being patientcentered and improving the safety of patient and staff. The current study was done to investigate the challenges of establishing the 3th edition of Iranian national accreditation standards in the hospitals affiliated to Islamic Azad University.Materials and methods: In this descriptiveapplied study, 239 employees (37 directors, 143 nursing staff and 59 physicians) participated in the study from two educational, research and therapeutic hospitals of BuAli and Amir almomenin. Nursing staff and physicians were selected by simple random sampling and managers by census method. The measurement tool was a researchermade questionnaire which designed by the Likert [completely agree (5) to completely opposed (1)] scoring system. The data were analyzed using ANOVA by SPSS. ver20 statistical software.Results: The most challenges of accreditation implementation from the viewpoint of participants were lack of personnel (87.87%), high number of standards and measures (81.17%), encountering the hospital's medical staff with heavy shift work (79.92%), the lack of budget (75.73%), and nonparticipation of physicians (62.76%). Conclusion: The results showed that the greatest challenge in implementing accreditation standards from the managers' and physicians' viewpoint was the lack of personnel and from the nursing staff's viewpoint was encountering the hospital's medical staff with heavy shift work.Keywords: Accreditation, Third edition of accreditation, Challenge.
