چکیده
|
هدف: بیماری پریکاردیت سلی فقط در یک تا دو درصد سل ها وجود دارد که از این نظر بسیار نادر است، از آنجایی که این بیماری عامل 4 درصد پریکاردیت حاد و 7 درصد تامپوناد های قلبی است لذا باتوجه به عوارض خطرناک این دو بیماری لذا تشخیص سریع و صحیح این بیماری از بروز این عوارض می تواند جلوگیری کند. شرح مورد: کیس گزارش شده پیرمرد 85 ساله ای است که با علائم بالینی تنگی نفس، صدای مبهم قلب و تورم ورید ژگولار معرفی شده است که در نوار قلب وی تاکی کاردی سینوسی و در یافته های آزمایشگاهی افزایش گلبول سفید (11.6) گزارش شده است. همچنین اقدامات تشخیصی دیگر مانند عکس قفسه سینه و اکو کاردیو گرفی انجام گردید که با تشخیص 28 میلی متر مایع پریکاردیال به صورت اورژانسی کاندید پریکاردیوتومی به صورت اورژانسی گردید،مایع تخلیه گردید و دارودرمانی و آنتی بیوتیک برای وی انجام گدید و نهایتا با حال عمومی خوب بیمار مرخص گردید. نتیجهگیری: در بررسی p cr (واکنش زنجیره ای پلی مراز) برای تشخیص سل، مایع پریکارد برای آزمایشگاه فرستاده شده که نتیجه مثبت اعلام شد، لذا با توجه به عوارض خطرناک این بیماری توصیه می شود برای تمامی بیماران پریکارد افیوژن شدید این آزمایش انجام شود.

Tuberculous Pericarditis Causing Severe Pericardial Effusion: A Case Study

Abstract
|
Aims Tuberculous pericarditis is present in only one to two percent of the tuberculosis cases, i.e. considered rare. The disease is responsible for 4% and 7% of acute pericarditis and cardiac tamponade cases, respectively. Moreover, these conditions are associated with hazardous side effects. Thus, timely and precise diagnosis of the disease could prevent such complications.Case report We reported an 85yearold male case who was referred with clinical signs of breath shortness, cardiac murmur, and swollen jugular veins. Besides, the relevant laboratory findings indicated elevated white blood cell counts (11.6). In addition, other diagnostic measures, such as thoracic radiography and echocardiography were performed on the patient. Then, the diagnosis indicated a 28mm diameter of pericardial fluid; accordingly, the patient was selected as a candidate for emergency pericardiotomy. The fluid was drained, and therapeutic medication, as well as antibiotics were administered for the patient. Finally, the patient was discharged with an appropriate general condition.Conclusion A sample of pericardial fluid was tested using polymerase chain reaction)method for the diagnosis of tuberculosis, which provided a positive result. Considering the dangerous complications of this disease, it is recommended for all patients who could suction severe pericardial effusion to be tested for this disease.
