چکیده
|
اهداف هدف از این مطالعه، مقایسه مقادیر کنتراست نسبی در سکانسهای سهبعدی dess با دو زاویه فلیپ 40 و 90 درجه با سایر سکانسهای روتین به منظور اثبات کارآمدی این سکانس در مقایسه با سایر سکانسهای رایج نظیر سکانس pd ،t2 ،stir و سکانس سهبُعدی flash در مشاهده بهتر ضایعات حاصل از ناهنجاریهای غضروفی در تصویربرداری تشدید مغناطیسی است.مواد و روش ها 15 بیمار که شامل 9 آقا (14-56 سال) و شش خانم (36-65 سال) که دارای علائم زانودرد بودند با معرفی پزشک ارتوپد تحت تصویربرداری تشدید مغناطیسی (mri) با استفاده از سکانسهای سهبُعدی dess با دو زاویه فلیپ 90 و 40 با سکانسهای pd t2 ،stir و سکانس سهبُعدی flash قرار گرفتند و تصاویر حاصل با استفاده از نرمافزار radiant dicom viewer 4.6.9 مورد آنالیز قرار گرفت و مقادیر کنتراست نسبی برای تصاویر ساژیتال انتخابشده در هر سکانس برای هر بیمار به دست آمد. آنالیز آماری دادههای بهدستآمده با استفاده از نسخه 16 نرمافزار آماری spss انجام شد. یافته ها تستهای آماری فریدمن و ویلکاکسون مقادیر معنادار p (کمتر از 0/05) را بین تمام سکانسهای موردبررسی در این مطالعه نشان دادند. سکانس dess با زاویه فلیپ 90 درجه با داشتن میانگین مقدار کنتراست نسبی 77/2667 دارای بهترین مقدار بود و در این بین سکانس flash با داشتن مقدار 3/733 دارای کمترین مقدارمیانگین کنتراست نسبی بود. نتیجه گیری سکانس dess با زاویه فلیپ 90 درجه دارای بهترین مقادیر کنتراست نسبی بود و بهترین سیگنال ممکن را از محل مایع مفصلی و نیز محل غضروف در تصویر فراهم کرد و بدین ترتیب امکان بررسی هرچه بهتر غضروف مفصلی را از نظر داشتن ضایعه مهیا میکند؛ بنابراین سکانس dess با زاویه فلیپ 90 درجه بهترین سکانس برای بررسی ناهنجاریهای غضروفی در بین سکانسهای مورد بررسی این مطالعه است.

Comparing Relative Contrast in Three-dimensional Double-echo Steady State With Routine Sequences for a Better Diagnosis of Knee Cartilage Anomalies

Abstract
|
Aims This study compared relative contrast values in threedimensional Double Echo Steady State (DESS) sequences with two flip angles of 40 and 90 degrees and other routine sequences. The obtained data could help to prove the effects of this sequence, compared to the other routine sequences; accordingly, it could be applied for better detection of lesions resulting from cartilage abnormalities.Methods Materials Total, 15 patients, including 9 men (1456 y) and 6 women (3665 y) with knee pain symptoms were referred to us by an orthopedic physician. All of the investigated patients underwent Magnetic Resonance Imaging (MRI) using DESS sequences with two flip angles of 40 and 90 degrees, as well as STIR , PD, T2, and 3D FLASH. We analyzed the acquired images using the Radiant DICOM Viewer4.6.9 software. We calculated relative contrast values for the selected SAGITAL images per sequence for each patient. The obtained data were analyzed using SPSS.Findings Friedman and Wilcoxon statistical tests data revealed significant P values (<0.05) for all the sequences examined in this study. The DESS sequence with a flip angle of 90 ° presented the most relative contrast value with the average score of 77.2667. The FLASH sequence provided the worst result of 3.733 for the relative contrast value.Conclusion The DESS sequence with 90 ° flip angle indicated the best relative contrast values and provided the best possible signal from the synovial fluid and the cartilage; therefore, it leads to the better examination of articular cartilage in terms of lesion. Accordingly, the DESS sequence with a flip angle of 90 ° was the optimal sequence to evaluate cartilage abnormalities among the examined sequences in this study.
