تبیین ادراک اعضای هیات علمی در زمینه چالش‌های موجود درآموزش پزشکی پاسخگو: یک تحلیل محتوای کیفی

زمینه و هدف: آموزش پزشکی به عنوان بخشی از نظام آموزش عالی در دهه های اخیر، با سرعت زیادی در حال توسعه و گسترش می باشد. با توجه به پیچیدگی های موجود در آموزش بالینی، تاثیرپذیری آن از عوامل مختلف محیطی، تغییرات اخیر در محیط بالینی، اهمیت پاسخگو بودن نسبت به بیماران و جامعه و چالش هایی که در زمینه آموزش پزشکی پاسخگو وجود دارد، به نظر می رسد که تبیین ادراک اعضای هیات علمی در رابطه با چالش های موجود در زمینه آموزش پزشکی پاسخگو که هدف این مطالعه بود، ضروری می باشد.روش کار: این مطالعه به روش تحقیق کیفی با رویکرد سنتی تحلیل محتوا در دانشگاه علوم پزشکی و خدمات بهداشتی درمانی شهید بهشتی در سال 1393 انجام گردید. دانش تولید شده در این رویکرد، بر اساس دیدگاه واحد و منحصر به فرد شرکت کنندگان و ریشه گرفته از داده های واقعی متن به دست آمد. مشارکت کنندگان، 8 نفر از اعضای هیات علمی بودند که بیشترین اطلاعات را در مورد عوامل زمینه ای اثرگذار بر آموزش پزشکی پاسخگو، بازگو نمودند (نمونه گیری هدفمند). مصاحبه های نیمه ساختارمند با آن ها صورت گرفت و از آن ها خواسته شد تا به توصیف تجربیات و ادراکات خود در خصوص آموزش پزشکی پاسخگو بپردازند. به این ترتیب، جمع آوری داده ها تا رسیدن به اشباع ادامه یافت. همزمان با جمع آوری داده ها، تجزیه و تحلیل آن ها نیز، ضمن در نظر داشتن معیارهای قابلیت اعتماد (trustworthiness) انجام شد.یافته ها: یافته ها شامل 4 درون مایه اصلی »آموزش معیوب و جامعه محور نبودن آن، مشکلات مدیریتی و کمبود منابع، عدم تعهد کافی در استادان به عنوان الگو (role model) و بی انگیزگی دانشجو « بود.نتیجه گیری: بر اساس نتایج مطالعه حاضر، ضرورت تغییرات و تحول بنیادین و همه جانبه در برنامه های آموزشی و ارتقای روند آن ها، به نحوی که نیازهای جامعه به صورت برجسته تری در آن ها نمایان باشد، به خوبی احساس شد. همچنین، راهکارهایی جهت افزایش انگیزه در استادان و دانشجویان ارایه گردید.

Exploring Faculty Members’ Perceptions about Socially Accountable Medical Education Challenges: A Qualitative Content Analysis

Background Objective: In recent decades, medical education has been expanding as part of the higher education system in the world. Due to the complexities of clinical education, the effect of various environmental factors on and changes in clinical environment, the importance of accountability to society and patients, and socially accountable medical education (SAME) challenges, the exploration of faculty members rsquo; perceptions about SAME challenges is necessary.Thus, the aim of this study was to explore faculty members rsquo; perceptions on the challenges of SAME.Methods: This qualitative study was conducted using qualitative content analysis in Shahid Beheshti University of Medical Sciences, Iran, in 2014. Knowledge obtained through qualitative content analysis is based on the unique views of the participants and actual data of the text. The participants were 8 faculty members who provided the most data on the fundamental factors effective on SAME (purposive sampling). Semistructured interviews were conducted with the participants and they were asked to describe their experiences of and views on SAME. Data collection was continued until achieving data saturation. Data collection and analysis were carried out simultaneously through consideration of trustworthiness.Results: The main themes extracted in this study were defective education which is not communitybased , lack of adequate commitment of teachers as role models , lack of motivation in students , and management problems and lack of resources .Conclusion: The results of the present study illustrate the necessity for fundamental and comprehensive modification of training programs and improvement of their processes, so that they can accommodate the community rsquo;s requirements. Moreover, there is need for strategies to increase motivation among teachers and students.
