بررسی پرونده‌های شکایت از متخصصین جراحی اعصاب ارجاعی به ادارۀ کمیسیون‌های پزشکی قانونی تهران 1393 – 1389

زمینه و هدف: یکی از مهم ترین مشکلات حرفه ای پزشکان، مسئلۀ نارضایتی و شکایت بیماران از آنان به مراجع قضایی و تبعات آن در زندگی شخصی و حرفه ای پزشکان است. خطاهای پزشکی ممکن است در هرجایی از نظام مراقبت بهداشتی اتفاق بیفتد. هدف از این مطالعه بررسی پرونده های قصور متخصصین جراحی اعصابِ ارجاعی به ادارۀ کمیسیون های پزشکی، قانونی تهران در یک دورۀ پنج سالۀ 1393 -1389 است.روش بررسی: در این مطالعۀ توصیفی مقطعی، همۀ پرونده های ارجاعی به ادارۀ کمیسیون های پزشکی قانونی در پنج سالۀ منتهی به سال 1393، ازنظر متغیرهای دموگرافیک و اختصاصی بررسی شدند.یافته ها: کل پرونده های شکایت از قصور پزشکی از ابتدای سال 1389 تا پایان سال 1393، برابر با 11370 پرونده است که تعداد 340 پرونده (3%) به قصور پزشکی رشتۀ تخصصی جراحی اعصاب مربوط می شود. محدودۀ سنی شاکیان، 1 تا 92 سال بود. 217 مرد و 123 زن جنسیت شاکیان را تشکیل می دادند. جنسیت جراحان اعصاب در این بررسی، 338 نفر مرد در مقابل 2 نفر زن بود. بیشترین علت شکایت از جراحان اعصاب، درگذشت بیمار، متعاقب جراحی بوده است. 127 اقدام درمانی، در مراکز دانشگاهی، 62 اقدام درمانی در مراکز دولتی غیردانشگاهی و 151 اقدام درمانی در مراکز خصوصی انجام گرفته بوده است. از 340 مورد پروندۀ شکایت از جراحان اعصاب، 255 تن (75%) از جراحان اعصاب تبرئه شدند و در 85 مورد (25%) جراحان اعصاب محکوم شدند.نتیجه گیری: میزان شکایات از متخصصین جراحی مغز و اعصاب طی سال های 1389 1393 رو به افزایش است. بین فراوانی قصور و خطاهای جراحان اعصاب با جنسیت جراح اعصاب ارتباط وجود دارد و جراحان اعصاب مذکر به مراتب مرتکب قصور پزشکی بیشتری نسبت به بانوان جراح اعصاب شده اند که می تواند ناشی از وجود تعداد قابل توجه آقایان جراح اعصاب نسبت به بانوان این رشته و نیز مراجعۀ بیشتر بیماران به جراح اعصاب مرد در کشور باشد.

Investigation Complaints from Neurosurgeons Referred to the Office of the Forensic Medicine Committees of Tehran

Background: One of the most important professional problems for doctors is the dissatisfaction and complaints of patients to the jurisdictions and the consequences in their personal and professional life. These medical errors may occur in different parts of the health care system. This study aimed to investigate failure cases of neurological surgeons referred to the Department of Forensic Medicine Commission in Tehran during a fiveyear period 20102014.Methods: In this descriptive crosssectional study, all cases which referred to Forensic Medicine Commission for five years ended in 2014, were evaluated for demographic and specific variables.Findings: The total number of complaints from medical failure from the beginning of 2010 to the end of 2014 is 11370, of which 340 cases (3%) were related to the failure of the medical specialty of neurosurgery. The range of ages was between 1 and 92 years old. Complainant consists of 217 males and 123 females. Neurosurgeons consist of 338 males and 2 females in this 5 year period. The most frequent cause of complaints was the death of the patient after surgery. 127 therapeutic interventions in University centers, 62 therapeutic interventions in governmental nonuniversity centers and 151 therapeutic interventions in private centers were done. Out of 340 complaints cases of neurological surgeons, 255 cases (75%) of nerve surgeons were acquitted and 85 (25%) were condemned.Conclusion: The total number of complaints from neurological surgeons during the years 20102014 is increasing. There is a correlation between the frequency of failures of neurogenic surgeons with the sex of the nerve surgeon, and male surgeons have more committed failure than female surgeons, which can be attributed to the presence of a significant number of male neurological surgeons for female in this field as well referring more patients to male neurosurgeons in the country.
