گزارش موردی از دایسکشن شریان ورتبرال به‌دنبال دستکاری گردن

اهداف: دایسکشن شریان های گردن (کاروتید و ورتبرال) به‌عنوان یکی از علل سکته مغزی شناخته شده است که اغلب به‌دنبال ترومای ناحیه گردن رخ می دهد. این مطالعه، گزارش موردی از دایسکشن شریان ورتبرال در یک خانم جوان به‌دنبال دستکاری گردن برای درمان سردرد توسط یک درمانگر محلی بود.بیمار و روش ها: بیمار، خانم 27ساله‌ای با سابقه سردردهای میگرنی بود که با افت هشیاری به بخش اورژانس بیمارستان مراجعه کرد. بررسی‌های اولیه متابولیک و عفونی منفی بودند، بنابراین بیمار تحت تصویربرداری مغزی قرار گرفت. سی‌تی‌اسکن اولیه، یافته خاصی نداشت. بنابراین ام‌آرآی انجام شد که نشان دهنده انفارکت حاد در پونز و مخچه بود. برای بررسی سیستم عروقی مغز، ام‌آر آنژیوگرافی انجام شد که انسداد شریان ورتبرال چپ را مشخص کرد و سی‌تی آنژیوگرافی تشخیص دایسکشن ورتبرال را به‌عنوان علت انسداد عروقی مسجل کرد. شرح حال دقیق بعداً مشخص کرد که بیمار قبل از مراجعه توسط یک درمانگر محلی مورد دستکاری گردن قرار گرفته است.نتیجه‌گیری: دایسکشن شریان های گردن به‌عنوان یکی از علل نقص نورولوژیک و کاهش سطح هشیاری باید در بیماران با ترومای جزئی یا شدید مد نظر باشد و زمانی که این تشخیص مطرح شد در شرح حال و معاینه به‌دنبال ترومایی بود که ممکن است اهمیت قانونی داشته باشد.

Case Report of Vertebral Artery Dissection Following Neck Manipulation

Aim(s): Neck arteries dissection (carotid and vertebral) is known as one of the causes of stroke which often occurs after a neck trauma. This study was a case report of a vertebral artery dissection in a young woman following a neck manipulation in order to treat a headache by a local therapist.Patient Information: The patient was a 27 years old woman with migraine headaches who referred to the emergency department with decreased consciousness. The results of metabolic and infectious examinations were negative, therefore brain imaging of the patient was considered. Initial CT scan had no particular finding. Therefore, MRI was performed which indicated acute infarction in the pons and cerebellum. MR angiography was performed to examine the vascular system of the brain which determined the left vertebral artery occlusion. CT angiography recognized vertebral artery dissection as the cause of vascular occlusion. Detailed examination of the patient's condition revealed that the patient experienced the neck manipulation by a local therapist before referral to the hospital.Conclusion: Neck arteries dissection as one of the causes of neurologic deficit and decrease the level of consciousness should be considered in patients with minor or severe trauma. After this diagnosis, the existence of trauma in the patient examination should be considered. Trauma may have a legal significance.
