میزان مواجهه شغلی کارکنان آزمایشگاه سمشناسی پزشکی قانونی بوشهر با حلالهای کلروفرم، دیاتیلاتر و آمونیاک

چکیده
|
اهداف: آمونیاک، کلروفرم و دیاتیلاتر سه حلال بسیار پرکاربرد در فرآیندهای آمادهسازی و استخراج نمونه در آزمایشگاههای پزشکی قانونی (بهخصوص آزمایشگاه سمشناسی قانونی) هستند و کارکنان این بخشها مواجهات قابلتوجهی را با این حلالها تجربه میکنند. مواجهه با این حلالها اثرات زیانباری مانند سمیتهای کبدی و عصبی، سرطانزایی و جهشزایی را میتواند به دنبال داشته باشد. هدف این مطالعه ارزیابی میزان مواجهه شغلی کارکنان آزمایشگاههای پزشکی قانونی بوشهر با این حلالها بود.مواد و روشها: این مطالعه تجربی بهصورت مقطعی در سال 1396 انجام شد. نمونهگیریها بهصورت فردی و در دو هفته متوالی به کمک پمپ نمونهبرداری فردی و لولههای جاذب صورت پذیرفت. تعیین مقدار کلروفرم و دیاتیلاتر با استفاده از دستگاه گازکروماتوگراف مجهز به دتکتور یونیزان شعلهای و آنالیز مقادیر آمونیاک با استفاده از دستگاه اسپکتروفتومتر در طولموج 660نانومتر انجام شد. میانگین وزنیزمانی مواجهه نیز به کمک فرمولهای مربوطه مورد محاسبه قرار گرفت.یافتهها: میانگین میزان مواجهه با کلروفرم، دیاتیلاتر و آمونیاک در بخش استخراج آزمایشگاه در روزهای بدون استخراج نمونه به ترتیب ppm 0/021، 0/007 و 0/17 و در روزهای استخراج به ترتیب ppm7/02، 32/25و 3/63 بود و بین میزان مواجهه کارکنان در روزهای استخراج و روزهای بدون استخراج تفاوت آماری معنیداری وجود داشت (p<0/05 ). میانگین وزنی زمانی مواجهه برای همه حلالها (بهجز کلروفرم) کمتر از حدود مجاز کشوری آنها بود.نتیجهگیری: اگرچه سطوح مواجههای کارکنان آزمایشگاههای پزشکی قانونی شهر بوشهر کمتر از حدود مجاز کشوری است، ولی در برخی موارد (بهخصوص در مورد حلال خطرناک کلروفرم) این مواجهات در سطح هشدار قرار دارد.

Abstract
|
Aims: Ammonia, chloroform, and diethyl ether are the three most commonly used solvents in sample preparation and extraction processes in forensic laboratories (especially forensic toxicology laboratory). The staffs of these labs are notably exposure to these solvents. Exposure to these solvents can have deleterious effects, such as hepatotoxicity, neurotoxicity, carcinogenicity and mutagenicity. The aim of this study was to evaluate the occupational exposure of Bushehr forensic laboratories staff to these solvents.Materials Methods: This experimental research was performed in a crosssectional study in 2017. Sampling was conducted individually for two consecutive weeks using individual sampling pump and adsorbent tubes. Analysis the amount of chloroform and diethyl ether was conducted using gas Chromatography with flameionization detector, and analysis the amount of ammonia was conducted using Spectrophotometer at 660 nm. Weighttime averages of exposure were also calculated using the relevant formulas.Findings: Weighttime averages of exposure to chloroform, diethyl ether and ammonia in the extraction section of the laboratory on days without sample extraction were 0.021, 0.0017 and 0.17ppm, respectively and on the days with sample extraction were 7.02 32.25 and 3.63ppm, respectively. There was a significant difference between the amount of staff exposure on the days of extraction and days without extraction (p<0.05). The weighttime average of exposure for all solvents (except chloroform) was less than their allowed limitation by the country.Conclusion: However, exposure levels of staffs of forensic medical laboratories in Bushehr is lower than the allowed limitation by the country, but in some cases (especially chloroform), these exposures are at the warning range.
