تحلیل دستور کارگذاری سیاست نظام ارجاع الکترونیک با استفاده از مدل جریان‌های چندگانه کینگدان

مقدمه: یکی از عوامل مهم برای اجرای مراقبت‌های اولیه بهداشتی، نظام ارجاع است. هدف از انجام این مطالعه، تحلیل دستور کارگذاری سیاست نظام ارجاع الکترونیک با استفاده از مدل جریان‌های چندگانه کینگدان در استان گلستان بود.مواد و روش‌ کار: این مطالعه کیفی در سال 1399 بر مبنای مدل کینگدان در استان گلستان انجام شد. داده‌ها با استفاده از دو روش بررسی اسناد و مستندات و مصاحبه‌های عمیق فردی با 26 مصاحبه فردی جمع‌آوری گردید. نمونه‌گیری به روش هدفمند و تحلیل اسناد و مستندات با روش تحلیل محتوا و تحلیل مصاحبه‌ها با روش تحلیل چارچوب انجام شد. یافته‌ها: جریان مشکل شامل تضعیف جایگاه پزشک عمومی و عدم مراجعه بیماران به آنها، افزایش پرداخت مردم و سازمان‌های بیمه‌گر و عدم حضور برخی پزشکان متخصص بود. جریان سیاست شامل الزام به اجرای برنامه پزشک خانواده و نظام ارجاع در قوانین و اسناد بالادستی، اجرای برنامه پزشک خانواده روستایی و نظام ارجاع در سال 1384 و برنامه پزشک خانواده شهری در استان‌های فارس و مازندران بود. جریان عزم سیاسی شامل راه‌اندازی سامانه ناب در دانشگاه علوم پزشکی گلستان، تصویب قانون تخصیص یک درصد از منابع ارزش افزوده به پیشگیری و درمان روستاییان در چارچوب نظام ارجاع، وجود سیاستگذاران کاریزماتیک و مسئولان اجرایی متعهد بود.نتیجه‌گیری: به رغم مشکلات ناشی از عدم اجرای نظام ارجاع و الزام قوانین به دلیل تعارض منافع و تامین مالی ناکافی اجرای سیاست‌ها در این زمینه، تلاقی سه جریان مشکل، سیاست و عزم سیاسی منجر به بازشدن پنجره سیاست و نظام ارجاع الکترونیک در استان گلستان گردید.

Analysis of electronic referral system agenda setting based on kingdon multiple streams framework

Objective (s): One of the important factors for the implementation of primary health care is the referral system. The aim of this study was to analyze of electronic referral system agenda setting based on Kingdon #39;s multiple streams framework in Golestan province.Methods: A qualitative study was conducted in 2020 based on Kingdan (2003) model in Golestan province. Data were collected using two methods of document review and indepth individual interviews with 26 individual interviews. Purposeful sampling and analysis of documents were performed by content analysis and interviews were analyzed by framework analysis.Results: The Problem stream was the weakening of the general practitioner and the nonreferral of patients, the increase in the payment of people and insurance organizations, and the absence of some specialist physicians. The Policy stream included the obligation to implement the family physician program and referral system in upstream laws and documents, the implementation of the rural family physician program and referral system in 2005, and the urban family physician program in Fars and Mazandaran provinces. The Politic stream included launching a lean system at Golestan University of Medical Sciences, passing a law allocating one percent of valueadded resources to the prevention and treatment of villagers within the referral system, and charismatic policymakers and committed executives.Conclusion: Despite the problems caused by the nonimplementation of the referral system and the enforcement of laws due to conflicts of interest and insufficient funding for policy implementation in this area, the intersection of three currents of problem, policy, and political determination led to the opening of the window of ereferral policy and system in Golestan province.
