ارزیابی خط‌مشی کاهش میزان پرداختی بیماران بستری در مراکز آموزشی درمانی دانشگاه علوم پزشکی استان گلستان

زمینه و هدف: در طرح تحول نظام سلامت، اولین بسته خدمتی برنامه‌ای برای کاهش میزان پرداخت بیماران بستری در بیمارستان‌های وابسته به وزارت بهداشت، درمان و آموزش پزشکی بود؛ بنابراین هدف مطالعه حاضر، ارزیابی خط‌مشی کاهش میزان پرداختی بیماران بستری در مراکز آموزشی درمانی دانشگاه علوم پزشکی استان گلستان بود.مواد و روش‌ها: پژوهش حاضر مقطعی از نوع توصیفی تحلیلی که در سال 1400 تا 1401 مورد بررسی قرار گرفته بود. نمونه آماری 384 نفر از کارکنان، کادر درمانی و پزشکان در مراکز آموزشی درمانی دانشگاه علوم پزشکی استان گلستان بودند که به‌صورت تصادفی ساده در این مطالعه شرکت کردند. ابزار جمع‌آوری داده‌ها پرسشنامه 35 سوالی، محقق ساخته بود که بعد از تایید روایی آن توسط خبرگان، اساتید راهنما و مشاور، پایایی آن با آزمون آلفای کرونباخ برآورد و عدد 0.83 به دست آمد که نشان‌دهنده پایایی پرسشنامه می‌باشد. داده‌ها با استفاده از نرم‌افزار spss22، آزمون‌های کلموگروف– اسمیرنوف و آزمون تی تک نمونه تحلیل شد.نتایج: در محور کاهش میزان پرداختی بیماران بستری در بیمارستان‌های وابسته به وزارت بهداشت، درمان و آموزش پزشکی طرح تحول نظام سلامت، بین وضعیت موجود و وضعیت مطلوب شکافی در حد 0.11- مشاهده می‌شود که با توجه به مجموع میانگین به‌دست‌آمده و این که بیانگر عدم دستیابی به اهداف اجرایی طرح تحول نظام سلامت در محور کاهش میزان پرداختی بیماران بستری است. نقطه تلاقی این دو وضعیت روی ماتریس بررسی شکاف، نشان‌دهنده شکاف در حوزه نظارتی می‌باشد.نتیجه‌گیری: بررسی نتایج حاکی از آن است که طرح تحول سلامت توانسته میزان پرداختی بیماران را کاهش دهد، ولی این اثربخشی در حد تحقق اهداف محور اول طرح نبوده است و در این زمینه ساماندهی روند بیمه همگانی و منابع مالی مورد نیاز ضروری می‌باشد.

evaluation of the policy on reducing out-of-pocket expenses for hospitalized patients in teaching hospitals of golestan university of medical sciences

background and purpose: as part of iran’s health system transformation plan (hstp), the first service package aimed to reduce out-of-pocket expenses for hospitalized patients in hospitals affiliated with the ministry of health and medical education. this study evaluates the effectiveness of the policy in reducing patient payments at teaching hospitals under golestan university of medical sciences.methods: this descriptive-analytical cross-sectional study was conducted between 2021 and 2022. a total of 384 participants—including healthcare workers, medical staff, and physicians—were randomly selected from teaching hospitals affiliated with golestan university of medical sciences. data were collected using a 35-item researcher-designed questionnaire, validated by experts, with a cronbach’s alpha reliability score of 0.83. data analysis was performed using spss version 22, kolmogorov-smirnov tests, and a one-sample t-test.results: indicate a gap of -0.11 between the current situation and the desired outcome of the hstp’s first axis (reducing patient payments). analysis using the gap analysis matrix revealed that the primary shortcoming is in regulatory oversight.conclusion: while the health system transformation plan has successfully reduced out-of-pocket expenses for hospitalized patients, it has not fully met the program’s intended financial relief targets. further policy adjustments are required, particularly in expanding universal insurance coverage and ensuring adequate financial resource allocation to improve accessibility and affordability for patients.
