موردپژوهی ایجاد بیمارستان غیردولتی در بخش دولتی؛ ارائه مدلی برای توسعه بخش غیردولتی نظام سلامت

زمینه و هدف: این مطالعه با هدف تبیین فرایند ایجاد یک بیمارستان غیردولتی و غیرانتفاعی به عنوان یک بیماستان شرکتی و ارائه مدلی برای ایجاد بیمارستانهای مستقل و شرکتی انجام شد.مواد و روش ها: مطالعه از نوع کیفی و موردی بود. داده ها با مصاحبه با بنیانگذاران بیمارستان محب مهر، کارکنان و برخی مسئولان دانشگاه و سازمانهای خیریه جمع آوری شد. روش تحلیل داده ها تئوری بنیادی بود و داده ها در سه مرحله باز، محوری و انتخابی کدگذاری شدند.نتایج: عوامل اولیه ایجاد بیمارستان محب مهر پایین بودن پرداختی پزشکان و اشتغال همزمان پزشکان در بخش دولتی و خصوصی بود. وجود فضاهای مرده در بیمارستان هاشمی نژاد و امکان قرارداد با بیمه های تکمیلی عوامل ثانویه بودند. واکنشها شامل استقبال پزشکان بیمارستان و مخالفت دولت و وزارت بهداشت بود. به دلیل پایین تر بودن تعرفه ها نسبت به بخش خصوصی و پذیرش بیمه های پایه و تکمیلی، دسترسی خوبی برای بیماران ایجاد شد. نظرات منفی دولت و وزارت بهداشت به تدریج تعدیل شد.نتیجه گیری: فرایند تمرکز زدایی بخش دولتی در بیمارستانها باید به صورت پایین به بالا صورت گیرد. بهتر است به جای تبدیل بیمارستان های دولتی به مستقل یا شرکتی، آنها را از نو به شکل مستقل یا شرکتی ایجاد کرد. همینطور قبل از هرگونه اقدام تمرکز زدایی، باید ثبات مالی و مدیریتی در سازمانها ایجاد شود. برای تمرکز زدایی در سیستم اداره بیمارستانها، منافع کارکنان باید حفظ شود. بهتر است به جای دریافت اعتبارات دولتی، از ظرفیت اوقاف و دریافت وام های بانکی استفاده شود

A case study on the establishment of nongovernmental hospitals in the public sector; providing a model for the development of the nongovernmental sector of health system

Background: This study aimed to explore the process of establishing a nongovernmental and nonprofit hospital, as a corporate infantry, and a model for the establishment of autonomous and corporate hospitals.Materials and Methods: This was a qualitative case study. The data were collected by interviewing the founders of Moheb Mehr Hospital, staff and some university officials and charity organizations. The data analysis was conducted based on grounded theory principles and data were coded in three stages of open, axial and selective coding.Results: The primary factors behind the establishment of Moheb Hospital were the low level of physicians' fees and the physicians rsquo; dual practice in the public and private sectors. The existence of unused spaces at HashemiNejad Hospital and the possibility of contracting supplementary insurance were the secondary factors. The reaction included welcoming from hospital physicians and opposition from the government and the Ministry of Health. A good patient access was created due to the lower charges, in comparison to the private sector, and having contract with the supplementary insurance. The negative comments from the government and the Ministry were gradually adjusted.Conclusion: The decentralization initiatives of the public sector in hospitals should be considered as a bottomup process. Instead of turning government hospitals into independent or corporate, it is better to reestablish them in an autonomous or corporate form. Also, before any decentralization action, financial and managerial stability must be created in the organizations. Morevoer, employee benefits must be preserved. Instead of receiving government funding, it is better to use the capacity of the endowment and receive bank loans.
