مقایسه بهای تمام شده خدمات واحد های رادیولوژی و سیتی اسکن در دو بیمارستان وابسته به دانشگاه علوم پزشکی اصفهان با تعرفه های مصوب و بررسی ظرفیت منابع با استفاده از روش هزینه یابی بر مبنای فعالیت زمان گرا

زمینه و هدف: محاسبه بهای تمام شده خدمات، موجب می شود بتوان منابع موجود را با کاراترین و موثرترین وجه ممکن به کار برد. هزینه یابی بر مبنای فعالیت زمان گرا، روشی است که علاوه بر هزینه یابی، میزان کارایی و ظرفیت بلا استفاده منابع را نیز محاسبه می کند. هدف از پژوهش، استفاده از این روش جهت محاسبه بهای تمام شده خدمات رادیولوژی و سیتی اسکن در دو بیمارستان بود.مواد و روش ها: پژوهش حاضر از نوع کمی است که به صورت مقطعی و گذشته نگر به شیوه توصیفی -تحلیلی در سال 1396 بر روی دو بیمارستان وابسته به دانشگاه علوم پزشکی اصفهان انجام گرفت. اطلاعات در قالب فرم های اکسل طراحی شده، جمع آوری و بهای تمام شده خدمات از طریق مدل هزینه یابی بر مبنای فعالیت زمان گرا (tdabc) محاسبه گردید.نتایج: بهای تمام شده اکثر خدمات رادیولوژی و کل خدمات سیتی اسکن در واحد دولتی بیشتر از واحد واگذار شده بود. ظرفیت بلا استفاده منابع نیز در بخش دولتی بیشتر از خصوصی بود. زمان انجام خدمات در دو بیمارستان به غیر از رده شغلی تکنسین در بقیه موارد تقریبا مشابه بود. میانگین بهای تمام شده خدمات رادیولوژی در بیمارستان (الف)، 223324 و در بیمارستان (ب)، 194094 ریال بود. میانگین بهای خدمات سیتی اسکن در بیمارستان (الف)، 600522 و در بیمارستان (ب)، 485230 ریال بود. نتیجه گیری: بر اساس یافته های مطالعه، نیاز به مدیریت منابع انسانی جهت کاهش ظرفیت بلا استفاده و افزایش کارایی و نیز بهبود فرآیندهای ارایه خدمت برای کاهش زمان ارایه خدمات ضروری به نظر می رسد. جایگزینی منابع با هزینه پایین راه حل دیگری است که در کاهش هزینه ها موثر می باشد. استانداردهای کاراترین بیمارستان ها و مراکز ارایه دهنده خدمات سلامت، اغلب ارزش این را دارند که سرمشق قرار گیرند

Comparing the costs of radiology and CT-scan services in two hospitals affiliated to Isfahan University of Medical Sciences with approved tariffs and studying resource capacity using Time- Driven Activity Based Costing (TDABC) Method

Background: Calculating the cost of services enables the existing resources to be used in the most efficient and effective way possible. Time Driven Activity Based Costing is a method that, in addition to costing, also calculates the efficiency and unused capacity of resources. The purpose of this study was to use this method to calculate the costs of radiology and CT scans services in two hospitals. Materials Methods: The present crosssectional and retrospective research was done in a descriptiveanalytic manner in two hospitals affiliated to Isfahan University of Medical Sciences in 2018. Data were collected in designed Excel forms, and costs of services were calculated using Time Driven ActivityBased Costing (TDABC) method. Results: The cost of most radiology services and total CT scan services in the government sector was more than that of the private sector. The unused capacity of resources in the public sector was more than private sector. The time spent for providing the services in the two hospitals was the same except for the technician's career. The average cost of radiology services was 223324 in hospital (A), and 194094 Rials in hospital (B). The average cost of the CT scan services was 600522 in hospital (A), and 485230 Rials in hospital (B). Conclusion: According to the findings of the study, the need for human resources management to reduce unused capacity and increase efficiency, as well as improving service delivery processes in order to reduce the time required to provide services seems necessary. Replacing lowcost resources is another solution that is effective in reducing costs. The standards of the most efficient centers of health services are often worth the benchmarking.
