مقایسه عملکرد بیمارستان شریعتی تهران قبل و بعد از طرح تحول نظام سلامت با استفاده از مدل تصمیم گیری چند شاخصه پویا

زمینه و هدف: طرح تحول سلامت از سال 1393 با هدف حفاظت مالی از مردم، ایجاد عدالت در دسترسی به خدمات سلامت و ارتقای کیفیت خدمات در بیمارستان ها اجرا شد. ارزیابی عملکرد بیمارستانها به منظور پایش طرح تحول سلامت ضروری است. یکی از ابزارهای ارزیابی عملکرد استفاده از روش پویای تصمیم گیری چندشاخصه می باشد. هدف از این تحقیق استفاده از روش تصمیم گیری چند شاخصه پویا به منظور ارزیابی عملکرد بیمارستان پیش و پس از طرح تحول نظام سلامت است.مواد و روشها: در این مطالعه مقطعی موردی، عملکرد بیمارستان شریعتی تهران طی 48 ماه متوالی( 24 ماه قبل و 24 ماه بعد از طرح تحول نظام سلامت،1394-1391) با استفاده از مدل تصمیم گیری چند شاخصه پویا مورد ارزیابی قرار گرفت که هرماه به عنوان یک گزینه در ماتریس تصمیم در نظر گرفته شد و با در نظر گیری 6 شاخص مهم با استفاده از روش ارزیابی محصول جمع شده با وزن به صورت پویا با کمک نرم افزار2010excel سنجیده شد.نتایج: نمرات عملکرد بیمارستان افت و خیزهایی در طی زمان داشت ولی به طور کلی یک روند افزایشی را در طول چهل و هشت ماه نشان داد. بیشترین و کمترین مقدار به ترتیب مربوط به ماه های 42 و 13 بود (0.4932و 0.9408). علاوه بر این در فروردین هر سال عملکرد بیمارستان کاهش داشت. نتیجه گیری: تغییرات نمرات عملکردی بیمارستان در طی بازه زمانی مورد بررسی حاکی از بهبود عملکرد بیمارستان می باشد. اما به طور قطع این روند افزایشی را نمی توان نتیجه مثبت طرح تحول نظام سلامت دانست. بنابراین، در عین توجه به بهبود مستمر روند کنونی، تداوم ارزشیابی و اصلاح عملکرد بیمارستان می تواند منجر به ارتقا عملکرد بیمارستان شود.

Comparison of Shariati hospital performance in Tehran before and after the implementation of health sector transformation plan using a dynamic multi-attribute decision-making method

Background: Iranian health system transformation plan has been implemented since 2014 with the aim of financial protection of people, equity establishment in access to health services and quality improvement of hospital services. Hospital performance evaluation is essential in order to monitor the health system transformation plan. One of the tools for evaluating hospital performance is the dynamic multiattribute decisionmaking method. The purpose of this study was to use the dynamic multiattribute decisionmaking method to evaluate the performance of the hospital before and after the implementation of the health system transformation plan.Materials and Methods: In this crosssectional study, the performance of Shariati Hospital in Tehran was evaluated for 48 consecutive months (24 months before and 24 months after the Health System Transformation plan, 20122016). Each month was addressed as an option in the decisionmaking matrix and ,with considering six important health indicators, the hospital performance was dynamically assessed using the weighted aggregated sum product assessment and through EXCEL software 2010. Results: Hospital performance scores had some fluctuations over time; however, in general, it had an increasing trend over fortyeight months. The highest and lowest values were for months 42 and 13 (0.4932 and 0.9408, respectively). In addition, in the first month of the solar year, the hospital performance score declined significantly for each year.Conclusion: Changes in hospital performance scores during the study period show improvement in hospital performance. Nevertheless, this incremental process cannot be considered as a positive outcome of the health system transformation plan. Therefore, while improving the current process, continuing the hospital performance evaluation can lead to the hospital's performance promotion.
