   تحلیل علل ریشه‌ای خطا در تشخیص آلودگی میکروبی بخش‌ مراقبت‌های ویژه یک بیمارستان منتخب دانشگاه علوم پزشکی تهران و ارائه راهکار مناسب با استفاده از تکنیک دلفی

چکیده    زمینه و هدف: آلودگی میکروبی بخش های بیمارستان از مهم ترین عوامل زمینه ساز انتقال عفونت های بیمارستانی در بیماران بستری در واحدهای بیمارستان به شمار می آید. این مطالعه باهدف تحلیل خطا در تشخیص آلودگی میکروبی بخش مراقبت های ویژه یک بیمارستان و تحلیل علل ریشه ای آن در سال 94 صورت گرفت.مواد و روش ها: این مطالعه در سه مرحله انجام شد. مرحله اول توصیف نمونه مورد مطالعه به صورت گزارش موردی بود. مرحله دوم به منظور شناسایی عوامل موثر بر بروز خطا با استفاده از تحلیل علل ریشه ای خطا و انجام مصاحبه و برگزاری پنل کارشناسی صورت گرفت. در مرحله سوم با استفاده از پرسشنامه خودساخته و به کارگیری تکنیک دلفی راهکارهای مناسب برای خطاهای شناسایی شده ارائه گردید. برای تجزیه و تحلیل داده ها از نرم افزار spss استفاده شد.نتایج: عمده ترین علل ایجاد عفونت بیمارستانی در بخش مراقبت های ویژه شامل علل مدیریتی و سازمانی، علل مرتبط با وظیفه، علل مرتبط با تجهیزات آزمایشگاه، علل مرتبط با آموزش، علل مرتبط با تیم کاری و علل مربوط به شرایط کاری بود. یافته های حاصل از تکنیک دلفی نشان داد که اقدامات ساده ای نظیر شستن دست و استفاده از لوازم یک بارمصرف به طور قابل ملاحظه ای در پیشگیری از عفونت بیمارستانی موثر هستند.نتیجه گیری: یکی از مهم ترین راه های پیشگیری و کنترل عفونت های بیمارستانی، بررسی علل ریشه ای این عفونت ها است. همچنین آموزش پرسنل در خصوص روش های موثر مراقبت و کنترل عفونت های بیمارستانی و فرهنگ ایمنی بیمار باید موردتوجه مدیران باشد.

   Root Causes Analysis (RCA) of Error in Detecting Microbial Contamination in Intensive Care Unit of a Selected Hospital Affiliated to Tehran University of Medical Sciences and Providing a Suitable Strategy Using Delphi Technique

Abstract    Background: microbial contamination of hospital wards is one of the most important factors in the transmission of hospital infections among inpatients in hospital units.This study aimed to analyze the error in the diagnosis of microbial contamination in the hospital's intensive care unit and its root causes analysis in 2015. Materials and Methods: This study was conducted in three stages. The first stage conducted as a case report. The second stage was designed to identify the factors affecting the occurrence of error by Root Causes Analysis and conducting an interview and panel. Third stage was provided using selfmade questionnaire and Delphi technique to obtain appropriate strategies to identified errors. In order to data analysis SPSS software was utilized. Results: The main causes of hospital infections were organizational and managerial causes, task related causes, laboratoryrelated causes, causes associated with training, causes associated with work teams and causes related to working conditions in the intensive care unit. The study findings extracted from the Delphi technique revealed that simple actions such as hand wash and disposable appliances usage are significantly effective in hospital infection prevention. Conclusion: One of the most important ways of preventing and controlling hospital infections is investigating the root causes of these infections. Also, personnel training regarding effective methods to nosocomial infections rsquo; care and control and patient safety culture should be considered by managers.
