مقایسه رادیوآناتومیکی شدت انحراف سپتوم بینی در بیماران مبتلا به پولیپوز سینونازال عودکننده و غیرعودکننده

زمینه و هدف: پولیپوزهای سینونازال یکی از اختلالات شایع دستگاه تنفسی است که سبب کاهش کیفیت زندگی بیمار و بروز عوارض ثانویه در آن ها می‌شود. مطالعه حاضر با هدف مقایسه میزان شدت انحراف سپتوم بینی در بیماران مبتلا به پولیپوز سینونازال عود کننده و غیر عود کننده طراحی شد. روش کار: مطالعه حاضر یک مطالعه مورد-شاهدی است که در فاصله سال‌های 1397 تا 1402 در بخش گوش-حلق و بینی بیمارستان غرضی اصفهان انجام گرفت. 80 بیمار مبتلا به پولیپوز سینونازال به دو گروه 40 تایی شاهد (بیماران دارای پولیپوز سینونازال غیر عود کننده) و مورد (بیماران دارای پولیپوز سینونازال عود کننده) تقسیم شدند. برای این منظور در نمونه سی تی اسکن قبل از جراحی، ساختارهای آناتومیکی بینی و سینوس‌های پارانازال مورد ارزیابی قرار گرفته و شدت انحراف سپتوم بینی با استفاده از نرم افزار osirix محاسبه شد.یافته ها: در گروه عود کننده میانگین زاویه انحراف سپتوم بینی 4.52±11.65 درجه و در گروه غیر عودکننده 3.76±9.7 درجه بود. بر اساس تست تی مستقل، بین دو گروه از نظر آماری تفاوت معناداری وجود داشت (0.039= p). همچنین نتایج نشان داد که ابتلا به آسم (0.043= p) و استعمال دخانیات (0.039= p) سبب تغییر معنی‌دار در گروه‌های مورد مطالعه می‌گردد. نتیجه گیری: نتایج نشان داد پولیپوز سینونازال عود کننده بیشتر از غیر عود کننده سبب انحراف شدید سپتوم می‌شود. اظهار نظر قطعی در مورد این موضوع منوط به بررسی دقیق تر با حجم نمونه بالاتر و همچنین بررسی ریسک فاکتورهای دخیل در عود مجدد پولیپوزها می‌باشد.     

radioanatomical comparison of the severity of nasal septum deviation in patients with recurrent and non-recurrent sinonasal polyposis

background: sinonasal polyposis is a common respiratory disorder that reduces the quality of life of patients and causes secondary complications. the present study was designed to compare the severity of nasal septal deviation in patients with recurrent and non-recurrent sinonasal polyposis.methods: the present study is a case-control study conducted between 2018 and 2023 in the ent department of gharazi hospital, isfahan. 80 patients with sinonasal polyposis were divided into two groups of 40: control (patients with non-recurrent sinonasal polyposis) and case (patients with recurrent sinonasal polyposis). for this purpose, the anatomical structures of the nose and paranasal sinuses were evaluated in the preoperative ct scan sample, and the severity of nasal septal deviation was calculated using osirix software.results: in the recurrent group, the mean angle of nasal septum deviation was 11.65±4.52 degrees and in the non-recurrent group, it was 9.7±3.76 degrees. based on the independent t-test, there was a statistically significant difference between the two groups (p=0.039). the results also showed that asthma (p=0.043) and smoking (p=0.039) caused significant changes in the studied groups.conclusion: the results showed that recurrent sinonasal polyposis causes severe septal deviation more often than non-recurrent. a definitive statement on this issue requires a more detailed study with a larger sample size and also an investigation of the risk factors involved in the recurrence of polyposis.
