چکیده
|
مقدمه: تنش واردشده بر بافتهای پیرامونی مفصل از طریق حرکت فعال مکرر ممکن است در نگهداری دامنه حرکتی به دست آمده در مفصل آرنج پس از متحرک سازی مفصلی نقش داشته باشد. اهداف: مقصود از مطالعه حاضر مقایسه اثرات متحرک سازی مفصلی با و بدون حرکت فعال مکرر بر دامنه حرکتی مفصل آرنج پس از بی حرکتی برای درمان شکستگی آرنج بود. مواد و روش ها: 28 شرکت کننده بادامنه حرکتی محدوددر مفصل آرنج پس از بی حرکتی برای شکستگی آرنج به طور تصادفی به گروه مداخله یاکنترل اختصاص داده شدند. گروه مداخله تکنیک های متحرک سازی maitland را یک روزدر میان و تمرینات حرکتی فعال مکرر را روزانه به مدت چهارهفته، علاوه برفیزیوتراپی معمول،دریافت کردند،در حالی که گروه کنترل تنها تکنیک های متحرک سازی maitland رادریافت کردند. دامنه های فعال خم شدن/باز شدن آرنج و چرخش داخلی/چرخش خارجی ساعد با بهره گیری از نرم افزار پروترکتوردرگوشی هوشمند قبل ازدرمان، حین درمان به طورهفتگی، و پس ازدرمان اندازه گیری شد. به علاوه، پرسشنامه ناتوانی بازو، شانه ودست قبل و بعد از مداخله چهارهفته ای نمره دهی شد. نتایج: دامنه همه حرکات اصلی مفصل آرنج در حین وپس ازمداخله درمقایسه با قبل ازمداخله درهردوگروه به طورمعنی داری افزایش یافت(0/001>p). به علاوه،گروه مداخله درمقایسه باگروه کنترل اختلاف معنی داری دردامنه خم شدن آرنج (0/001 >p ) و چرخش داخلی ساعد (0/036 = p ) نشان داد. نمرات پرسشنامه ناتوانی بازو، شانه ودست درهردوگروه به طورمعنی داری بهبود یافت(0/001>p). نتیجه گیری: مطالعه حاضر نشان داد که حرکت فعال مکرر روزانه در دامنه حرکتی محدود مفصل آرنج پس از بی حرکتی برای درمان شکستگی آرنج به طور معنی داری دامنه به دست آمده در خم شدن آرنج و چرخش داخلی ساعد را از طریق متحرک سازی مفصلی حفظ می نماید.

Abstract
|
background: the stress exerted on periarticular tissues through repeated active motion may contribute to preserving the acquired range of motion in the elbow joint after joint mobilization.objectives: the purpose of the present study was to compare the effects of joint mobilization with and without repeated active motion on the restricted range of motion in the elbow joint after prescribed immobilization for treating an elbow fracture.methods: twenty-eight participants with a restricted range of motion in the elbow joint after prescribed immobilization for their elbow fracture were randomly assigned to either the intervention or control group. the intervention group received maitland's mobilization techniques every other day. it repeated active motion exercises daily for four weeks, in addition to routine physical therapy, while the control group received only maitland's mobilization techniques. the active ranges of elbow flexion/extension and forearm supination/pronation were measured using smartphone protractor software before, weekly during, and after the intervention. additionally, the disability of the arm, shoulder, and hand questionnaire was scored before and after the four-week intervention.results: the range of all cardinal motions in the elbow joint significantly (p < 0.001) increased during and after the intervention compared to before the intervention in both groups. additionally, the intervention group exhibited a significant difference in the range of elbow flexion (p < 0.001) and forearm pronation (p = 0.036) compared to the control group. the disability of the arm, shoulder, and hand questionnaire scores significantly improved in both groups (p < 0.001).conclusions: the present study revealed, for the first time, that daily repeated active motion within a restricted range of motion in the elbow joint after prescribed immobilization for treating an elbow fracture significantly maintains the acquired range of elbow flexion and forearm pronation through joint mobilization.
