تحلیل روند بروز و مرگ‌ ومیر ناشی از آلزایمر و سایر دمانس‌ها طی 30 سال‌ اخیر در ایران

هدف: ارزیابی روند بیماری آلزایمر و سایر دمانس‌ها به علت عدم درمان قطعی، افزایش امید به زندگی و پیری جمعیت از اهمیت زیادی برخوردار است. این مطالعه با هدف بررسی روند طولانی‌مدت از میزان بروز و میزان مرگ‌ومیر ناشی از آلزایمر و سایر دمانس‌ها در ایران طی سال‌های 981369 انجام شده است.مواد و روش‌ها: میزان بروز استاندارد شده سنی (agestandardized incidence rate: asir)، میزان مرگ‌ومیر استاندارد شده سنی (agestandardized mortality rate: asmr) آلزایمر و سایر دمانس‌ها به تفکیک جنسیت، میزان بروز و مرگ‌ومیر برای هر گروه سنی 5 ساله برای کشور ایران از سایت بار جهانی بیماری‌ها (global burden of disease: gbd) استخراج شد. روندهای asir و asmr با استفاده از مدل رگرسیون نقطه اتصال (joinpoint regession) مورد تجزیه و تحلیل قرار گرفت. یافته‌ها: در این مطالعه روند asir آلزایمر و سایر دمانس‌ها طی 30 سال به طور میانگین سالانه 02/0% افزایش معنی‌دار و asmr 004/0 % کاهش غیر معنی‌دار سالانه مشاهده شد. روند asir و asmr آلزایمر و سایر دمانس‌ها در طی دوره بررسی برای مردان نزولی اما برای زنان صعودی بود و بیش‌ترین میانگین درصد تغییرات سالانه (average annual percent change: aapc) میزان بروز و مرگ‌ومیر ناشی از آلزایمر و سایر دمانس‌ها به ترتیب مربوط به گروه سنی 5450 سال با aapc برابر 19/0 و گروه سنی 7975 سال با aapc برابر 11/0 مشاهده شد.نتیجه‌گیری: نتایج این مطالعه نشان داد که در ایران asir و asmr آلزایمر و سایر دمانس‌ها در مردان از روند نزولی و در زنان از روند صعودی برخوردار بوده است و میزان آن‌ها نسبت به منطقه بیش‌تر است. بنابراین باید اطلاع‌رسانی و اقدامات پیشگیرانه‌ی جدی‌تری به ویژه برای زنان و گروه سنی 5450 سال صورت گیرد.

Analysis of the incidence and mortality rate of Alzheimer's and other dementias during the last 30 years in Iran

Introduction: Assessing the course of Alzheimer #39;s disease and other dementias is very important due to lack of a definitive treatment, increase in life expectancy, and aging population. This study aimed to investigate the longterm trend in the incidence and mortality rate of Alzheimer #39;s and other dementias in Iran during 19902019.Materials and Methods: AgeStandardized Incidence Rate (ASIR) and AgeStandardized Mortality Rate (ASMR) of Alzheimer #39;s and other dementias was collected by sex, and incidence and mortality rates of Iran for each 5 yeas group was extracted from the Global Burden of Disease site. ASIR and ASMR trends were analyzed using the Joinpoint Regression model.Results: In this study, the trend of standardized incidence of agerelated Alzheimer #39;s and other dementias increased significantly by an yearly average of 0.02% for 30 years, and for the mortality rate, 0.004% annual insignificant decrease was observed. The trend in the standardized incidence and mortality of Alzheimer #39;s and other dementias during the study period was declining for men but upward for women. The highest average annual percent change (AAPC) for incidence and mortality of Alzheimer #39;s and other dementias was observed in the age group of 5054 years with AAPC= 0.19, and 7579 years with AAPC= 0.11 respectively.Conclusion: The results of study showed that in Iran the ASIR, and ASMR of Alzheimer #39;s and other dementias is decreasing in male and increasing in femals, and also its rate is more in the area. Therefore, informative action, and more serious preventive steps should be taken, especially for women and in the age group of 5054 years.
