   رابطه افسردگی و آگاهی از دیابت با میزان خودمراقبتی بیماران مبتلا به دیابت نوع دو

چکیده    هدف: شواهد نشان می‌دهد خودمراقبتی در دیابت، یکی از مهم‌ترین را‌ه‌کارهای اساسی جهت کنترل بیماری محسوب می‌شود. از آن‌جا که رابطه آگاهی و افسردگی با خودمراقبتی بیماران مبتلا به دیابت قندی نیازمند بررسی می‌باشد، هدف این مطالعه تعیین رابطه افسردگی و آگاهی از دیابت با میزان خودمراقبتی بیماران مبتلا به دیابت نوع دو بود.مواد و روش‌ها: در این مطالعه مقطعی 145 نفر از بیماران مبتلا به دیابت قندی نوع دو بستری در بیمارستان کوثر شهر سمنان مورد بررسی قرار گرفتند. جمع‌آوری داده‌ها با پرسش‌نامه‌های اطلاعات دموگرافیک، افسردگی beck، آگاهی از دیابت (adknowl) audit of diabetes knowledge و خودمراقبتیsummary of diabetes selfcare activities (sdsca) انجام شد.یافته‌ها: 20.7% بیماران افسردگی خفیف، 42.1% افسردگی متوسط و 13.8% افسردگی شدید داشتند. به طور کلی 76.6% بیماران درجه‌ای از افسردگی (نمره ده یا بالاتر) داشتند. وضعیت تاهل (0.004=p)، سطح تحصیل (0.003=p)، محل زندگی (0.021=p)، وضعیت منزل مسکونی (0.045=p) و سن (0.002=p) با شدت افسردگی ارتباط معنی‌داری داشتند. میانگین ± انحراف معیار آگاهی 2.8 ±14.5 بود. متغیرهای جنس (0.027=p)، وضعیت تاهل (0.027=p)، سطح تحصیل (0.009=p)، وضعیت شغلی (0.043=p) با آگاهی ارتباط معنی‌داری داشتند. میانگین ± انحراف معیار خودمراقبتی 5.9 ±23.1 بود. خودمراقبتی با هیچ یک از مشخصات دموگرافیک ارتباط معنی‌دار نداشت. بین افسردگی و آگاهی (0.077= r،0.3555=p)، افسردگی و خودمراقبتی (0.050= r،0.551=p) و آگاهی و خودمراقبتی (0.065= r،0.434=p) ارتباط معنی‌دار دیده نشد.نتیجه‌گیری: یافته‌های مطالعه نشان‌دهنده شیوع بالای افسردگی، آگاهی متوسط و خودمراقبتی کم در بیماران دیابت قندی نوع دو می‌باشد، از آن‌جا که آگاهی و افسردگی با خودمراقبتی رابطه معنی‌دار نداشتند، باید عوامل مرتبط با خودمراقبتی بیماران دیابت قندی نوع دو مورد بررسی گسترده‌تری قرار گیرد، تا با شناسایی عوامل مرتبط با آن، بتوان در جهت کنترل بیماری و عوارض آن اقدام نمود.

   Relationship between depression and knowledge about diabetes with the amount of self-care in patients with type 2 diabetes

Abstract    Introduction: Evidence suggests that selfcare diabetes is one of the most important strategies for controlling the disease. Since it is required to assess the relationship between knowledge and depression with selfcare in patients with diabetes mellitus, the aim of this study was to determine the relationship between depression and knowledge about diabetes with the amount of selfcare in patients with type 2 diabetes.Materials and Methods: In this crosssectional study, 145 patients with type 2 diabetes mellitus were studied in Semnan Kowars Hospital. Data collection was performed with questionnaires demographic information, Beck depression, Audit of Diabetes Knowledge (ADKnowl) and Summary of Diabetes SelfCare Activities (SDSCA).Results: 20.7% of patients had mild depression, 42.1% had moderate depression and 13.8% had severe depression. Overall, 76.6% of patients had a degree of depression (score of 10 or higher). The severity of depression had a significant relationship with marital status (P=0.004), education level (P=0.003), place of living (P=0.021), residential status (P=0.045) and age (P=0.002). The mean ± SD of knowledge was 14.5 ±2.8. Knowledge have a significant relationship with the variables sex (P=0.027), marital status (P=0.027), education level (P=0.009), occupational status (P=0.043). Mean ± SD of selfcare was 23.1 ±5.9. Selfcare had no significant relationship with any of the demographic characteristics. There were not a significant correlation between depression and knowledge (r=0.077, P=0.3555), depression and selfcare (r=0.050, P=0.551) and selfcare and knowledge (r=0.065, P=0.434).Conclusion: The findings of this study showed a high prevalence of depression, moderate knowledge and low selfcare in patients with type 2 diabetes mellitus. Since knowledge and depression were not significantly correlated with selfcare, the factors related to selfcare in patients with type 2 diabetes mellitus should be studied more widely, so that the identification of its related factors can be used to control the disease and its complications.
