گزارش یک مورد نادر هایپرپلازی پاپیلاری التهابی بدون دنچر

هدف: هایپرپلازی التهابی پاپیلارییا (inflammatory papillary hyperplasia, iph) ضایعه خوش خیم مخاطی است که معمولاً کام سخت را در افرادی که از دنچر (دست دندان) استفاده می کنند، درگیر می کند. اکثر بیماران از وقوع آن ناآگاه هستند اما بعضی سوزش و ناراحتی دارند. نشان داده شده است که iph با استفاده 24 ساعته از دنچر، به خصوص در شب ارتباط دارد. علی رغم پاتوژنز اصلی ناشناخته از آن جایی که استفاده از دنچرهای باتطابق نامناسب و قدیمی و تکثیر میکروارگانیسم ها ازعلل اصلیiph دانسته شده است و طرح ریزی درمان ها بر این اساس گذاشته شده، این مقاله به توصیف و گزارش یک مورد نادر از iph در بیماری فاقد دنچر پرداخته است.گزارش مورد: بیمار خانم 29 ساله از نظر سیستمیک سالم، به بخش بیماری های دهان دانشکده دندان پزشکی دانشگاه شهید صدوقی یزد جهت معاینه دندان دردناک مراجعه نمود. درمعاینه داخل دهانی روتینیک پلاک با سطح pebbly، قرمز، با حدود نامشخص و نامنظم در فرورفتگی غیر نرمال موجود در کام مشاهده شد. دهان شویه نیستاتین تجویز شد. بررسی دوباره یک هفته پس از استفاده از دارو انجام شد.سایز ضایعه و اریتم سطحی کاهش یافته و به طور کلی ضایعه بهتر شده بود. به علت عدم ایجاد علائم و ناراحتی بیمار در جلسات فالوآپ بعدی شرکت نکرد.نتیجه گیری: iph همیشه در ارتباط با دنچر نیست وکام عمیق و ابنورمالیتی های کام نیز می تواند از علل بروز این ضایعه باشد که نیازمند درمان های تیمی توسط پزشکان و دندان پزشکان جهت برطرف شدن کامل این مشکل می باشد.

Denture free inflammatory papillary hyperplasia: A rare case report

Introduction: Inflammatory papillary hyperplasia (IPH) is a benign mucosal lesion that frequently affects palatal mucosa in denture wearers. Notably, most of the cases are asymptomatic but some of the patients feel the burning sensation and discomfort. In this way, IPH is associated with 24hours use of denture especially at night, so the main treatment modalities focused on denture modification and candidiasis infection inhibition. Current article reports a rare case of IPH in a denture free person.Case report: A 29 years old healthy woman referred to department of oral medicine, School of Dentistry Shahid Sadoughi University of Medical Sciences (Yazd, Iran) with complaint of toothache. In our routine intraoral examination, a red pebbly plaque with irregular and poorly defined borders on palatal vault was inspected. Nystatin suspension was advised. Reexamination was done after one week that reduction in lesion size and erythema was observed and the lesion was improved generally. Unfortunately, she didn’t represent on next followup session for no signs and discomfort.Conclusion: IPH is not always associated with dentures, but also deep and abnormal palate could be the cause of this lesion, requiring multidisciplinary approaches between physicians and dentists’ especially oral medicine to resolve this problem completely.
