   تاثیر تمرینات تناوبی شدید بر سطوح پلاسمایی آیریزین و شاخص‌های گلایسمی در افراد چاق و دیابتی نوع دو: مرور نظام‌مند و فراتحلیل

چکیده    مقدمه: آیریزین، مایوکاین جدیدی است که از پروتئین غشایی fndc5 آزاد شده و تاثیرات مثبتی بر متابولیسم دارد. هدف از پژوهش حاضر، بررسی اثر تمرینات تناوبی شدید بر سطوح پلاسمایی آیریزین و شاخص‌های گلایسمی در افراد چاق و دیابتی نوع دو بود.روش‌ها: جستجوی سیستماتیک مقالات انگلیسی و فارسی منتشر شده از پایگاه‌های اطلاعاتیpubmed ،web of science ، scopus، sid و magiran تا 30 آبان‌ماه 1403 (20 نوامبر2024) انجام شد. تفاوت میانگین و فاصله اطمینان 95 درصد (ci) با استفاده از مدل اثر تصادفی محاسبه شد. ناهمگونی با استفاده از آزمون (i2) و سوگیری انتشار با تحلیل بصری فونل پلات و آزمون egger بررسی گردید.یافته‌ها: در مجموع 11 مطالعه و 264 آزمودنی چاق و دیابتی نوع دو وارد فراتحلیل حاضر شدند. نتایج نشان داد تمرین تناوبی شدید موجب افزایش معنادار آیریزین ]001/0p= ، (616/0 الی280/2) ng/ml 448/1 wmd =‌‌[، کاهش معنادار گلوکز ]049/0p= ، (180/24- الی 074/0-) mg/dl 127/12- =‌[wmd و مقاومت به انسولین ]001/0p= ، (983/1- الی 567/0-) 275/1- =‌[wmd  نسبت به گروه شاهد در افراد چاق و دیابتی نوع دو شد. در حالی که انسولین ]894/0p= ، (901/3- الی 402/3)  iu/ml249/0- = [wmd کاهش معناداری نداشت.نتیجه‌گیری: روی هم‌رفته یافته‌های فراتحلیل حاضر نشان می‌دهد که تمرین تناوبی شدید نقش موثری در آیریزین، گلوکز، انسولین و مقاومت به انسولین به تمرینات تناوبی شدید ایفا می‌کند. تمرین تناوبی شدید، در افراد چاق و دیابتی موجب افزایش آیریزین، کاهش گلوکز، انسولین و مقاومت به انسولین می‌شود. اصلاح سبک زندگی و تمرین ورزشی، یک راهبرد درمانی ترجیحی در افراد چاق و دیابتی نوع دو است.

   the effect of high-intensity interval training on plasma irisin levels and glycemic indices in obese and type 2 diabetic people: a systematic review and meta-analysis

Abstract    background: irisine is a new myokine that is released from the membrane protein fndc5 and has positive effects on metabolism. the aim of the present study was to investigate the effect of high-intensity interval training on plasma irisin levels and glycemic indices in obese and type 2 diabetic people.methods: a systematic search of published english and persian articles was conducted from pubmed, web of science, scopus, sid, and magiran databases up to 20 november 2024. the mean difference and 95% confidence interval (ci) were calculated using a random-effects model. heterogeneity was assessed using the (i2) test and publication bias was assessed using visual analysis of funnel plots and egger’s test.results: a total of 11 studies and 264 obese and type 2 diabetic subjects were included in the present meta-analysis. the results showed that high-intensity interval training significantly increased irisin [wmd = 1.448 ng/ml (2.280 to 0.616), p= 0.001], significantly decreased glucose [wmd = -12.127 mg/dl (-24.180 to -0.074), p= 0.049] and insulin resistance [wmd= -1.275 (-0.567 to -1.983), p= 0.001] compared to the control group in obese and type 2 diabetic subjects. while insulin [wmd= -0.249iu/ml (3.402 to -3.901), p= 0.894] was not significantly reduced.conclusion: overall, the findings of the present meta-analysis indicate that high-intensity interval training plays an effective role in irisin, glucose, insulin, and insulin resistance to high-intensity interval training. high-intensity interval training increases irisin, decreases glucose, insulin, and insulin resistance in obese and diabetic individuals. lifestyle modification and exercise training are preferred treatment strategy in obese and type 2 diabetic people.
