اپیدمیولوژی و بار اقتصادی بیماری‌های اسکلتی-عضلانی در سربازان و کارکنان ارتش

مقدمه: مشکلات اسکلتی-عضلانی از منظر اقتصادی، توانمندی و سلامتی و بهداشت موضوع بسیار مهمی در ارتش است. این مطالعه به‌منظور بررسی توزیع انواع بیماری‌های اسکلتی-عضلانی و سپس برآورد هزینه‌های مستقیم و غیرمستقیم ناشی از این شکایات و ارائۀ راه حل برای بهبود و تصحیح شرایط سربازان و کارکنان ارتش انجام شد.روش‌ها: کلیۀ بیماران مراجعه کننده با شکایات اسکلتی-عضلانی در کمیته‌های روماتولوژی، اورتوپدی و جراحی مغز و اعصاب بیمارستان نظامی از سال 1393 تا 1395 مورد بررسی قرار گرفتند. هزینۀ مراحل تشخیصی براساس داده‌های سوابق محاسبه شد و هزینۀ درمان برای هر شکایت با محاسبۀ میانگین هزینۀ طرح‌های درمانی پیشنهادی دو متخصص طب فیزیکی و روماتولوژی برآورد شد. هزینه‌های غیرمستقیم برآورد و به هزینۀ مستقیم اضافه شد. تجزیه‌و‌تحلیل آماری با spss نسخۀ 21 انجام شد.یافته‌ها: 2116 بیمار با میانگین سنی 8.17 ± 25.84 سال مورد بررسی قرار گرفتند. 1252 نفر (59.16%) سرباز و 864 نفر (40.83%) غیر سرباز بودند. سه مورد از شایع‌ترین شکایت، شامل شکستگی (301 مورد، 14.22%)، برآمدگی دیسک کمر و فتق (303 مورد، 14.31%) و ژنوواروس/ژنو والگوس (257 مورد، 12.14%) بودند. اندام تحتانی و ستون مهره‌ها بیشترین آسیب را داشتند. در مجموع سالانه 4120 نفر-روز غیبت در این افراد برآورد شد و سالانه 903.120/1.713 تومان (معادل 928/ 40.727 دلار آمریکا) اثر اقتصادی سالانه محاسبه شد.نتیجه گیری: بیماری‌های اسکلتی عضلانی یکی از معضلات سلامتی و بهداشتی واقتصادی درارتش است. ضروری است استفادۀ صحیح از روش‌های تشخیصی و درمانی و توجه به قانون cost & benefit برای کاهش بار اقتصادی و از کارافتادگی از الویت‌های دانشگاه ارتش برای پیشگیری و صرفه‌جویی در هزینه سلامت باشد.

the frequency and financial impact of musculoskeletal disorders in military

background: we aimed to examine the distribution of all types of musculoskeletal diseases and then to estimate the direct and indirect costs caused by these complaints among soldiers and non-soldiers’ employees in military.methods: we recruited all patients referred with musculoskeletal complaints who were admitted to the rheumatology, orthopedics and neurosurgery committees of 501 army hospital from 2013 to 2015. the cost of diagnostic procedures was calculated based on the data of records and the cost of treatment for each complaint was estimated by calculating the average cost of treatment plans proposed by two physical medicine and rheumatology specialists. indirect costs due to leave and reduced working time and hospitalization and disability were estimated and added to the direct cost.results: 2116 patients with an average age of 25.84 ± 8.17 years were examined. 1252 people were soldiers and 864 people were non-soldiers. fracture (301 cases), lumbar disc bulges and herniation (303 cases), and geno varus/geno valgus (257 cases) were three most common complaints. the most damaged site were lower limbs and spine. in total, 4,120 person-days of absence were estimated for these people annually, and the annual economic effect was calculated to be 1,713,903,120 tomans.conclusion: musculoskeletal diseases are one of the prevalent health and economic problems in military. therefore, application of diagnostic and therapeutic strategies and considering the cost & benefit law to reduce the economic burden and disability should be among the priorities of the army university to prevent and save health costs.
