طراحی و بومی‌سازی نرم‌افزار ثبت سیستمی دستورهای پزشک در ایران طی سال‌های 1394-1392

زمینه و هدف: نرم افزار ثبت سیستمی دستورهای پزشک از فناوری های نوین در راستای افزایش کیفیت خدمات بیمارستانی است. مطالعه حاضر با هدف طراحی و بومی سازی نرم افزار ثبت سیستمی دستورهای پزشک در دانشگاه علوم پزشکی شیراز انجام شد.مواد و روش ها: این مطالعه اکتشافی با هدف طراحی نرم افزار و به صورت کاربردی در سال 1394-1392 در بیمارستان نمازی شیراز انجام شد. جامعه پژوهش شامل پزشکان، پرستاران و متخصصین فناوری اطلاعات دانشگاه علوم پزشکی شیراز بود. این مطالعه در چهار مرحله انجام شد؛ در سه مرحله اول برای جمع آوری داده های موردنیاز نرم افزار، از روش بررسی مستندات، دلفی و جلسات بحث گروهی متمرکز استفاده شد و در مرحله چهارم نرم افزار طراحی شد.یافته ها: نرم افزار ثبت سیستمی دستورهای پزشک در مدت 13 ماه طراحی گردید و اجزای اصلی آن شامل ورود و ثبت اطلاعات، سیستم مدیریت تداخلات دارویی، سیستم اخطاردهی، خدمات درمانی، قابلیت تایپ، گزارش گیری و توانایی های فنی بود. مهم ترین ویژگی های این نرم افزار امکان ارتباط پزشک و پرستار، اتصال نرم افزار به سیستم اطلاعات بیمارستان و استفاده از سیستم پشتیبان تصمیمات بالینی بود. نرم افزار در بخش مراقبت های ویژه عمومی بیمارستان نمازی اجرا شد و در مدت 3 ماه، ثبت دستورهای پزشکی برای 97 بیمار از طریق آن انجام شد.نتیجه گیری: نرم افزار ثبت سیستمی دستورهای پزشک در بیمارستان طراحی و اجرا شد. برای اجرای موفقیت آمیز نرم افزارهای بیمارستانی باید نظرات کاربران نهایی مورد توجه قرار گیرد. ارتباط بیشتر سیستم پشتیبان تصمیم یار بالینی با نرم افزار ثبت دستورهای پزشک پیشنهاد می شود.

Design and Localization of Computerized Physician Order Entry System in Iran during 2013-2015

Background and Objectives: Computerized Physician Order Entry (CPOE) is one of the modern technologies to increase the quality of hospital services.The present study was designed to develop and localize CPOE in Shiraz University of Medical Sciences.Materials and Methods: This exploratory study was conducted for software designing and was practically performed between 2013 to 2015 in Namazi Teaching Hospital in Shiraz. The study population consisted of physicians, nurses, and information technology professionals in Shiraz University of Medical Sciences. The study included four phases; in the first three phases documentations review, Delphi, and focused group discussions methods for data collection and CPOE software was designed in the fourth stage.Results: The CPOE software was designed in 13 months and had seven main including data entry, drug interactions management system, warning system, treatment services, ability of writing in the software, software reporting and technical capabilities. The most important features of this software were the possibility of physiciannurse relationship, software connection with the Hospital Information System (HIS) and use of Clinical Decision Support Systems (CDSS). The software was implemented in the general Intensive Care Unit (ICU) ward and used for physicians rsquo; order registration for three months.Conclusion: The CPOE was designed and implemented in Namazi Hospital. The comments of operators have to be considered for successful application of any software in hospital.
