تاثیر 8 هفته تمرینات اصلاحی بر درد، دامنه حرکتی و زاویه کرانیوورتبرال بیماران مبتلا به میگرن همراه با سر به جلو: یک مطالعه نیمهتجربی

چکیده
|
زمینه و هدف: میگرن یکی از اختلالات عصبی است و معمولاً وضعیت سر و گردن با سردرد میگرن همراه است. لذا هدف از این مطالعه تعیین تاثیر 8 هفته تمرینات اصلاحی بر درد، دامنه حرکتی و زاویه کرانیوورتبرال بیماران مبتلا به میگرن دارای ناهنجاری سر به جلو میباشد. مواد و روشها: مطالعه حاضر از نوع نیمهتجربی با طرح پیشآزمون-پسآزمون با گروه کنترل بود که بر روی 26 نفر از مبتلایان مبتلا به میگرن همراه با ناهنجاری سر به جلو در شهر یزد در سال 1403 انجام گرفت که بهطور تصادفی به گروه تمرین (12 نفر) و کنترل (14 نفر) تخصیص داده شدند. گروه تمرین 8 هفته تمرینات اصلاحی را انجام دادند و گروه کنترل زندگی عادی خود را داشتند. درد با پرسشنامه mcgill و دامنه حرکتی با استفاده از گونیامتر اندازهگیری گردید. دادهها با استفاده از آنالیز کوواریانس چندمتغیره تجزیه و تحلیل شد.یافتهها: نتایج نشان داد، 8 هفته تمرینات اصلاحی بر مولفههای درد افراد دارای میگرن با ناهنجاری عضلانی-اسکلتی گردن تاثیر دارد (0.001>p) که بیشترین تاثیر بر مولفه دردهای متنوع با کاهش 28.65 درصدی بود. همچنین، این تمرینات بر دامنه حرکتی چرخشی گردن (0.001>p) تاثیر داشت. زاویه کرانیوورتبرال نمونهها با انجام این تمرینات افزایش معنیدار داشت (0.001>p).نتیجهگیری: اصلاح ناهنجاری سر به جلو و متعاقب آن اصلاح وضعیت بدنی میتواند موجب کاهش درد، بهبود دامنه حرکتی عضلات در ناحیه گردن بیماران دارای میگرن گردد. لذا پیشنهاد میشود تمرینات اصلاحی برای ناهنجاریهای سر به جلو در مداخلات درمانی برای این بیماران دارای میگرن لحاظ گردد.واژههای کلیدی: میگرن، سر به جلو، حرکات اصلاحی، درد، ناهنجاری

Abstract
|
background and objectives: migraine is one of neurological disorders and is often associated with head and neck posture. therefore, the aim of this study was to determine the effect of 8 weeks of corrective exercises on pain, range of motion, and the craniovertebral angle in migraine patients with forward head posture.material and methods: this quasi-experimental study with a pre-test/post-test design and a control group was conducted on 26 migraine patients with forward head posture in yazd, iran, in 2024. participants were randomly divided into two groups included 12 in the exercise group and 14 in the control group. the exercise group performed 8 weeks of corrective exercises including strength and stretching exercises, while the control group continued their normal daily activities. pain was measured using the mcgill pain questionnaire, and range of motion was assessed using a goniometer. data were analyzed using multivariate analysis of covariance (mancova).results: the results indicated that 8 weeks of corrective exercises significantly affected pain components in migraine patients with musculoskeletal neck abnormalities (p<0.001), with the greatest impact on pain perception, showing a 28.65% reduction. additionally, these exercises significantly improved the rotational range of motion of the neck (p<0.001). the craniovertebral angle of the participants also showed a significant increase following the exercises (p<0.001). conclusion: correcting forward head posture and subsequently improving body posture can reduce pain and enhance the range of motion in the neck muscles of migraine patients. therefore, it is recommended that corrective exercises for forward head posture be included in therapeutic interventions for migraine patients.keywords: migraine, forward head posture, corrective exercises, pain, deformityfunding: this study was funded by yazd university.conflict of interest: none declared.ethical considerations: the ethics committee of yazd university approved the study (ir.yazd.rec.1403.071).authors’ contributions:- conceptualization: zeinab karimi, saeed abedinzadeh - methodology: saeed abedinzadeh, hamid abbasi, reza sharifatpour- data collection: zeinab karimi, reza sharifatpour- formal analysis: hamid abbasi, reza sharifatpour- supervision: saeed abedinzadeh- project administration: saeed abedinzadeh- writing – original draft: zeinab karimi, hamid abbasi, reza sharifatpour- writing – review & editing: saeed abedinzadeh, hamid abbasi, reza sharifatpour
