افزایش ریسک اختلالات کلیوی در بیماران مبتلا به کروناویروس 2019: نامه به سردبیر

زمینه و هدف: بیماری کرونا ویروس 2019 (کووید-19) به یک بیماری عالم‌گیر تبدیل شده است که می‌تواند اندام‌های مختلف بدن انسان را درگیر نماید. اختلالات کلیوی در بیماران مبتلا به کووید-19 اغلب شایع است. درگیری در کلیه‌ها به صورت پروتئینوری و آسیب حاد کلیه (acute kidney injury; aki) ظاهر می‌شود. این ویروس می‌تواند از طریق آنزیم مبدل آنژیوتانسین 2 (angiotensin converting enzyme 2; ace2) منجر به نکروز حاد توبولی، نشت پروتئین در کپسول بومن، گلومرولوپاتی و اختلال میتوکندری شود. این ویروس هم‌چنین می‌تواند باعث اختلال در تنظیم پاسخ‌های ایمنی (از جمله طوفان سایتوکین)، فعال شدن نامتعادل سیستم رنین-آنژیوتانسین، فعال شدن نامتعادل ماکروفاژها در کلیه‌ها شود. از آنجایی که تا به امروز استراتژی درمانی اختصاصی برای محافظت از کلیه‌ها در برابر کووید-19 وجود ندارد. لذا با توجه به نقش حیاتی کلیه‌ها در حفظ هموستاز بدن، پیشنهاد می‌گردد در طول درمان بیماران مبتلا به کووید-19 توجه ویژه‌ای به این اندام گردد.
