مقایسه تاثیر الکترواکوپانکچر، لیزر کم‌توان و قرص سلوککسیب در درمان استئوآرتریت مفصل زانو: یک مطالعه کارآزمایی بالینی یک سو کور

زمینه و هدف: زانو بیشتر از مفاصل دیگر مبتلا به استئوآرتریت می شود. علت این بیماری ناشی از تغییرات ساییدگی است که به تدریج اتفاق می افتد. هدف این مطالعه تعیین تاثیر الکترواکوپانکچر، لیزر کم توان و قرص سلوککسیب در درمان استئوآرتریت زانو بود.مواد و روش ها: این مطالعه به صورت کارآزمایی بالینی یک سوکور در درمانگاه فیریوتراپی رفسنجان در سال های 1395 و 1396 انجام گرفت. شصت بیمار مبتلا به استئوآرتریت زانو به طور تصادفی به سه گروه مساوی 20 نمونه ای تقسیم شدند و تحت بررسی سه روش الکترواکوپانکچر، لیزر و قرص سلوککسیب به مدت 10 جلسه قرار گرفتند. ارزیابی درد، حرکات زانو و زمان تست 6 دقیقه ای راه رفتن به ترتیب از طریق مقیاس آنالوگ بصری، گونیا متر و زمان سنج طی جلسات اول، پنجم، دهم و 1 ماه پس از پایان درمان بررسی گردید. داده ها با استفاده از تحلیل واریانس دو طرفه با اندازه گیری مکرر، آزمون مقایسات چندگانه tukey و مجذور کای، تجزیه و تحلیل شدند.یافته ها: نتایج این مطالعه نشان داد الکتراکوپانکچر از لیزر و قرص سلوککسیب به مدت 10 جلسه در کاهش درد زانو و افزایش زمان تست 6 دقیقه ای موثرتر بود (0.001>p). در رابطه با بهبودی حرکات زانو نیز الکترواکوپانکچر از لیزر و قرص سلوککسیب تاثیر بیشتری داشت (0.001>p).نتیجه گیری: بر اساس یافته های این مطالعه، به نظر می رسد الکترواکوپانکچر به مدت 10 جلسه می تواند در کاهش درد، بهبودی حرکات و افزایش تست 6 دقیقه ای از لیزر و قرص سلوککسیب موثرتر باشد.

Comparing the Effects of Electroacupunture, Low Level Laser and Celecoxib Tablet on Treatment of the Knee Joint Osteoarthritis: A Randomized Single-Blinded Clinical Trial

Background and Objectives: Knee is affected by osteoarthritis more than any other joints. The cause of this disease is degeneration changes that occur gradually. The aim of this study was to compare the effects of Electroacupunture (EA), Low Level Laser Therapy (LLLT) ,and Celecoxib Tablet (CT) on treatment of knee osteoarthritis.Materials and Methods: This randomized singleblinded clinical trial was performed in Fatemieh Physiotherapy Clinic of Rafsanjan during 20162017. Sixty patients conflicted to osteoarthritis of knee were randomly divided into three equal groups (each group with 20 samples) . The first, second ,and third groups received EA, LLLT .and CT, respectively during 10 sessions. Intensity of pain, movement, and the time of 6 Minute Walking Test (6MWT)were assessed during the first, fifth, and tenth sessions and 1 month after the end of treatments. Pain intensity, movements ,and the time of 6MWT were evaluated by Visual Analogue Scale (VAS), goniameter ,and chronometer, respectively. Data were analyzed using twoway repeated measures ANOVA followed by Tukey rsquo;s multiple comparisons test and chisquare test.Results: The findings showed that 10 sessions of EA was more effective than LLLT and CT in reduction of pain in knee osteoarthritis (p<0.001). Also in relation to knee joint movements, EA was more effective than LLLT and CT in this joint (p<0.001). The finding of this study showed that EA was more effective than LLLT and CT in increasing the time of 6MWT (p<0.001).Conclusion: According to this study, it seems that during 10 sessions, EA might be more effective than LLLT and CT in reducing pain, improving movement ,and increasing the time of 6MWT inknee osteoarthritis.Key words: Electroacupunture, Low Level Laser, knee osteoarthritis, Celecoxib Tablet Funding: This research was funded by Rafsanjan University of Medical Sciences.Conflict of interest: None declared.Ethical approval: The Ethics Committee of Rafsanjan University of Medical Sciences approved this study.
