بی‌اختیاری ادرار ناشی از تجویز ترازودون در دوره یائسگی: گزارش یک مورد

ترازودون یک مهار کننده ضعیف بازجذب سروتونین و آنتاگونیست قوی گیرنده های سروتونینی 5ht2a و 5ht2c است. بی اختیاری ادرار ناشی از داروهای ضدافسردگی عارضه نادری است. عوارض جانبی ناشی از داروهای ضدافسردگی می تواند به تاخیر در بهبودی و در نهایت عدم کمپلیانس بیماران منجر شود. بیمار خانم 63 ساله یائسه و متاهل بود که سابقه افتادگی مثانه و مصرف داروهای کوئیتیاپین و پروپرانولول را از قبل داشت؛ اما سابقه هیچ نوع بیماری جسمی، مصرف مواد و بی اختیاری ادرار را قبل از تجویز دارو نداشت. بیمار به علت افسردگی، اضطراب و بی خوابی با تجویز روانپزشک شروع به مصرف قرص ترازودون به میزان 50 میلی گرم شبانه نمود. پس از آن به صورت ناگهانی دچار بی اختیاری ادرار گردید و به علت عدم قطع شدن عارضه، مجبور به قطع دارو گردید. پس از قطع دارو عارضه برطرف گردید. پس از قطع بی اختیاری ادراری بیمار و بهبودی کامل، مجدداً داروی ترازودون با همان دوز 50 میلی گرم برای خواب شروع شد و بلافاصله پس از تجویز دارو بیمار مجدداً دچار بی اختیاری ادراری گردید. در نتیجه این دارو را بایستی یک عامل مهم ایجاد کننده بی اختیاری ادرار در نظر گرفت و با احتیاط اقدام به تجویز آن نمود.

Trazodone induced urinary incontinence in woman at menopause period: A case report

Trazodone is a weak inhibitor of serotonin reuptake and a potent antagonist of serotonin 5HT2A and 5HT2C receptors. Urinary incontinence is a rare side effect of antidepressant drugs. Side effects of antidepressants can cause delay in treatment of patient and ultimately noncompliance in patients who take this medicinal drug. Here we present a case of trazodone induced urinary incontinence in a woman at menopause period. The patient was a retired woman with 63 years old. She reported a history of bladder prolapsuse and history of consumption of quetiapine and propranolol. But the patient had no history of other somatic illnesses, drug and urinary incontinence before administering the medicinal drug. Psychiatrist prescrided trazodone with dose of 50 mg, orally per day when appearence of the depression, anxiety and insomnia. But, the patient suddenly suffered from urinary incontinence, despite this situation the patient had continued taking the drug for three days. Due to the uncertainty of the above complications patient, was forced to discontinue the drug and follwoing drug discontinuation the side effect disappeard. After the improvement of the urine incontinence and full recovery, retreatment was began with the same dose of 50 mg trazodone for the patient, but immediately after drug administration to the patient, urinary incontinence appeared once again. Trazodone can be considered as important factor to cause urinary incontinence. Therefore, this issue should be considered whenever this pharmaceutical drug is prescribed.
