عوارض بارداری و زایمان در زنان باردار مبتلا به covid-19

سابقه و هدف: با توجه به این‌که زنان باردار در هنگام شیوع بیماری‌های عفونی در گروه پرخطر قرار می‌گیرند، هدف از این مطالعه بررسی عوارض بارداری و زایمان در زنان باردار مبتلا به کووید 19 بود. مواد و روش‌ها: در این مطالعه مقطعی که از سال 1398 تا 1400 انجام شد، 100 زن باردار که برای بررسی شکایت تب، تنگی نفس، سرفه یا اسهال به بیمارستان امام خمینی ساری مراجعه یا اعزام شده بودند و با شک به پنومونی ناشی از covid-19  (با معیارهای بیوشیمیایی و تصویربرداری)، بستری و تحت درمان قرار گرفته بودند، بررسی شدند. یافته‌ها: شدت بیماری کووید در پیک‌های مختلف در زنان باردار آلوده از نظر آماری تفاوت معنی‌داری داشت (p=0/002)، به طوری که 71 درصد از این افراد در پیک سوم، درگیری شدیدتری با کووید داشتند. بستری شدن به دلیل پره‌ترم لیبر (p=0/019) و اختلال مایع آمنیوتیک (p=0/004) در پیک دوم به ‌طور معنی‌داری، بیش‌تر از سایر پیک‌ها بود. در پیک دوم، میزان مایع آمنیوتیک غیر طبیعی در سه ماهه سوم بیش‌تر از پیک‌های دیگر بود (p=0/05). 66/7 درصد از ختم بارداری‌ها در موارد شدید کووید به علت مشکلات مادری بود که به طور معنی‌داری بیش‌تر از موارد جنینی بود (p=0/043). استنتاج: با توجه به خطر بالای عواقب مادری و نوزادی کووید 19 در دوران بارداری، باید در این دوران اقدامات احتیاطی ویژه برای جلوگیری از ابتلا به این بیماری انجام شود.

background and purpose: considering that pregnant women are among the high-risk groups during the outbreak of infectious diseases, the purpose of this study is to investigate the complications of pregnancy and childbirth in pregnant women who were infected with (covid-19). materials and methods: in this cross-sectional study, which was conducted from 2020 to 2022, 100 pregnant women who were referred or sent to imam khomeini hospital in sari due to complaints ,such as fever, shortness of breath, cough or diarrhea and were suspected of having pneumonia caused by covid-19 (according to biochemical and imaging criteria) were admitted to the hospital and treated, and the complications of pregnancy and childbirth were investigated. results: in the infected pregnant women, there was a statistically significant difference in the severity of the covid disease in different peaks (p=0.002), such that 71% of these individuals had more severe involvement of covid disease in the third peak. hospitalization for preterm labor (p=0.019) and amniotic fluid disorders (p=0.004) were significantly more common in the second peak than in other peaks. in the second peak, the amount of abnormal amniotic fluid in the third trimester was higher than in the other peaks (p=0.05). 66.7% of reasons for termination of pregnancy in severe cases of covid-19 were due to maternal problems, which was significantly more than fetal cases (p=0.043). conclusion: due to the high risk of maternal and neonatal consequences of covid-19 during pregnancy, special precautions should be taken during this period to prevent infection with this disease.
