ارتباط بین استئوآرتریت و استئوپروز در جمعیت بیماران مراجعه کننده به بیمارستان های شهرستان ساری

سابقه و هدف: استئوآرتریت و استئوپروز از مهمترین عوامل ایجادکننده اختلالات اسکلتی بوده و ارتباط آن‌ها با یکدیگر و سایر عوامل، موضوعی بحث برانگیز و مجهول می‌باشد. مطالعه حاضر با هدف بررسی ارتباط بین استئوآرتریت و استئوپروز و نیز ارتباط بین این دو بیماری با مشخصات دموگرافیکی و سبک زندگی بیماران انجام پذیرفت.مواد و روش‌ها: مطالعه مقطعی حاضر به روش سرشماری بر روی بیمارانی که به بیمارستان‌های شهرستان ساری در 3 ماهه چهارم سال 1397 مراجعه کرده بودند، انجام گرفت. شدت استئوآرتریت پس از رادیوگرافی از مفصل زانو، براساس معیار kellgrenlawrence مشخص گردید. اندازه‌گیری سطوح bmd نیز با روش dxa scan انجام شد. ابزار جمع‌آوری اطلاعات شامل پرسشنامه دموگرافیکی و ابعاد سبک زندگی بود. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار spss18 انجام گرفت.یافته‌ها: تعداد 77 بیمار مورد مطالعه قرار گرفتند که میانگین سن بیماران 10/3 ±57/3 سال و 73 نفر (94/8 درصد) زن بودند. از لحاظ شدت استئوآرتریت، 28 بیمار (36/36 درصد) نوع خفیف، 31 بیمار (4026/26 درصد) نوع متوسط و 18 بیمار (23/38 درصد) نوع شدید بودند. میانگین bmd در بیماران نیز 0/06 ±0/76 گرم بر سانتی‌متر مربع محاسبه شد. بین شدت استئوآرتریت و مقدار bmd اختلاف معنادار آماری وجود داشت (0/008= p). بین این دو بیماری با سن، جنس، وزن و برخی از شاخص‌های سبک زندگی اختلاف معنادار آماری مشاهده شد (0/05 >p).استنتاج: استئوآرتریت در مفصل زانو با بروز استئوپروز ارتباط دارد. در این راستا عوامل دموگرافیکی و سبک زندگی می‌توانند به عنوان مهم‌ترین عوامل تاثیرگذار در بروز این بیماری‌ها باشند.

The relationship between Osteoarthritis and Osteoporosis in the Relationship between Osteoarthritis and Osteoporosis in A Population of Patients Attending Sari Hospitals

Background and purpose: Osteoarthritis and osteoporosis are the most important causes of skeletal disorders and there are controversies over the relationship between these diseases and other factors. The aim of the present study was to investigate the relationship between osteoarthritis and osteoporosis and also the association between these two diseases and patients rsquo; demographic and lifestyle characteristics.Materials and methods: This crosssectional study was performed in patients attending Sari hospitals in the fourth quarter of 2019 selected via census sampling. Knee radiography was performed and the severity of osteoarthritis joint was determined based on KellgrenLawrence criterion. Bone mineral density (BMD) was also measured by DXA scan. Data was collected using demographic questionnaire and lifestyle dimensions. Data were analyzed in SPSS V18.Results: A total of 77 patients were studied. The mean age of patients was 57.3 ±10.3 years and the majority were females (n= 73, 94.8%). Severity of osteoarthritis was determined as mild in 28 (36.36%), moderate in 31 (40.26%), and severe in 18 (23.38%) patients. The mean BMD level was 0.76 ±0.06 g/cm2. There was a significant difference between the severity of osteoarthritis and BMD level (P=0.008). Significant differences were found between patients with osteoarthritis and osteoporosis in age, sex, weight, and some lifestyle indicators (P<0.05).Conclusion: Osteoarthritis of the knee joint is associated with osteoporosis. But, demographic and lifestyle characteristics could have a major role in the incidence of these diseases.
