مقایسه اثر داروی ترانکزامیک اسید و دسموپرسین وریدی در کاهش میزان خونریزی هنگام عمل سپتورینوپلاستی

سابقه و هدف: خونریزی یکی از مشکلات رایج در بیماران تحت عمل جراحی سپتورینوپلاستی است که می تواند روی سیر و شرایط بیماران در هنگام و پس از عمل موثر باشد. بنابراین، این مطالعه برای بررسی و مقایسه اثر دو داروی ترانکزامیک اسید و دسموپرسین وریدی در کاهش میزان خونریزی هنگام عمل سپتورینوپلاستی انجام شده است.مواد و روش ها: در این مطالعه مداخله ای که به صورت یک کارآزمایی بالینی تصادفی انجام شد، تعداد 105 نفر از بیماران تحت عمل جراحی سپتورینوپلاستی در بیمارستان فیروزگر و حضرت فاطمه تهران در سال 1398 به صورت در دسترس انتخاب شدند، به صورت تصادفی در یکی از سه گروه ترانکزامیک اسید و دسموپرسین وریدی و دارونما تقسیم شدند، میزان خونریزی و رضایت مندی تیم جراحی و تغییرهای همودینامیک در سه گروه تعیین شده و مقایسه شد.یافته ها: میزان رضایت مندی در گروه دسموپرسین بیش از دو گروه ترانکزامیک و دارونما بود (p=0.0001). میزان خونریزی در بیماران بررسی شده در گروه دسموپرسین کمتر از دو گروه ترانکزامیک و دارونما بود (p=0.0001). تغییرهای همودینامیک در سه گروه اختلاف آماری نداشت.نتیجه گیری: ترانکزامیک اسید و دسموپرسین وریدی اثربخشی خوبی در کاهش میزان خونریزی هنگام عمل سپتورینوپلاستی دارند که به خصوص در مورد دسموپرسین بیشتر هم است.

Comparison of the Effects of Tranexamic Acid and Intravenous Desmopressin on Blood Loss and the Quality of the Surgical Field during Septorhinoplasty

Introduction: Bleeding is one of the most common problems in patients undergoing septorhinoplasty, which can affect the course and condition of patients during and after surgery. Therefore, the present study was conducted to investigate and compare the effects of Tranexamic acid and intravenous Desmopressin on reducing the amount of bleeding during Septorhinoplasty.Method: In the current randomized doubleblind clinical trial, 105 patients undergoing Septorhinoplasty in Firoozgar Hospital and Hazrat Fatemeh Hospital in Tehran in 2019 were selected as available sampling and randomly divided into three groups: Tranexamic acid group, intravenous desmopressin, and placebo. The blood loss and satisfaction of the surgical team and hemodynamic changes were determined and compared in the three groups.Results: The level of satisfaction in the desmopressin group was more than those of the two groups of tranexamic acid and placebo (P = 0.0001). Also, blood loss in the desmopressin group was lower than those of the other two groups (P = 0.0001). Hemodynamic changes in the three groups were not observed to be statistically significant.Conclusion: Tranexamic acid and intravenous desmopressin were more effective in reducing bleeding during septorhinoplasty, especially in the case of desmopressin.
