بررسی شکایت‌ها از درمان ایمپلنت دندانی و عوامل مرتبط با آن در مراجعان به اداره کمیسیون‌های پزشکی قانونی تهران(1397-1395)

سابقه و هدف: با توجه به بررسی مطالعه های انجام شده روی شکایت بیماران از درمان های دندان پزشکی، با توجه به اینکه شکایت بیماران از درمان های ایمپلنت دندانی کمتر انجام شده است، این تحقیق براساس شکایت بیماران از درمان ایمپلنت و عوارض ناشی از آن در سال های 1395-1397 انجام شد.مواد و روش ها: این تحقیق به روش توصیفی و با مراجعه به اداره کمیسیون های پزشکی قانونی تهران و دریافت مجوز مربوطه انجام شد. پرونده تمامی دندان پزشکانی که در سال های 1395-1397 به اداره کمیسیون های پزشکی قانونی تهران ارجاع شده بود، بررسی شد. از بین آن ها پرونده های مربوط به شکایت بیماران از درمان ایمپلنت جدا شد. در این پرونده ها عوامل مرتبط از جمله سن وجنس بیمار، دلیل شکایت و خصوصیات دندان پزشک مورد شکایت از نظر جنس، سن و تخصص بررسی و ثبت شد. رای کمیسیون پزشکی به صورت تبرئه یا قصور مشخص و شیوع هر کدام از این دو رای در نمونه ها تعیین و میزان واقعی آن برآورد شد و سپس نقش عوامل مرتبط با آزمون کای دو قضاوت آماری شد.یافته ها: از کل شاکیان دندان پزشکی در این سال ها (1395-1397)،حدود 30 درصد از پرونده ها مربوط به شکایت از درمان ایمپلنت بود به طوری که 58 درصد ناشی از جایگزینی نامناسب ایمپلنت، 29 درصد ناشی از آسیب به بافت های اطراف ایمپلنت، 6 درصد مربوط به اخذ هزینه های بالا و 7 درصد شامل سایر موارد بود. در این پرونده ها، شاکیان به تفکیک 50 درصد مرد و 50 درصد زن و دندان پزشکان به تفکیک 91 درصد مرد و 9 درصد زن بودند. رای کمیسیون پزشکی قانونی در 45 درصد موارد تبرئه و 55 درصد رای قصور بود.نتیجه گیری: به نظر می رسد، میزان قصور دندان پزشکی در درمان ایمپلنت بالا بوده و به خصوص میزان قصور برای دندان پزشکان فاقد تخصص بیشتر دیده می شود. نتایج این تحقیق اقدام های لازم برای کاهش این مشکل را توصیه می کند.

Evaluation of dental complaints in implants treatment and related factors referred to department of Tehran legal medical committee (20162018)

Background and aim: According to the study of patient’s complaints about dental treatments and the fact that patients’ complaints have been less about dental implant treatment, this study aimed to investigate the dental complaints in implants treatment and related factors during 20162018. Materials and methods: Following a descriptive method, the relevant permissions were obtained after referring to the Forensic Medical Commission of Tehran. The files of all the dentists were evaluated during 2016 until 2018. Then, the files related to the complaints about tooth implantation services were separated. The patient relevant factors were evaluated and documented such as age, gender, and the reason of complaint, as well as dentist related factors including age and gender. Afterwards, the judgements of the commission were determined including exoneration or obvious negligence and the frequency of each judgement in the samples. The real judgement frequency (CI) was estimated and the roles of relevant factors were statically determined using the Chi Square Test.Results: Amongst all dental services complainers, onethird (30%) of the files were related to the tooth implantation services during 20162018, that included inappropriate implant installation (58%), damage to the implant surrounding tissue (29%), high service costs (6%), and other problems (7%). Also, the complainers were 50% women and 50% men and the dentists were 9% women and 91% men. Moreover, the judgements of Forensic Medical Commission were exoneration in 45% of the cases and negligence (55%) in the remaining cases.Conclusion: This rate of failure in implant services was high, especially for nonspecialized dentists. The results of the present study suggest taking necessary actions to reduce the problem.
