تحلیل خط‌مشی‌گذاری در نظام سلامت بر اساس الگوی چارچوب ائتلاف مدافع (مطالعه موردی طرح تحول نظام سلامت)

در این مطالعه تلاش شده با استفاده از الگوی «چارچوب ائتلاف مدافع»، تغییر و شکل‏گیری خط‏مشی طرح تحول نظام سلامت مورد بررسی قرار گیرد. برای این منظور از دو روش کتابخانه‌ای و مصاحبه استفاده شده است. برای انتخاب مصاحبه‌‏شوندگان از روش گلوله برفی و در مقام تجزیه و تحلیل داده‌های مصاحبه، روش تحلیل تم استفاده شده است. نتایج حاصل از تحلیل مصاحبه‏‌های نیمه‏‌ساختاریافته با 12 نفر از کارشناسان و خبرگان نظام سلامت، نشان‌‏دهنده توانایی چارچوب ائتلاف مدافع در توضیح فرآیند تغییر خط‏مشی نظام سلامت بوده و بر این اساس، می‌‏توان چهار مرحله در شکل‌گیری طرح تحول را به صورت زیر تبیین کرد: 1. اتفاق‏نظر و اجماع ملی؛ 2. تغییر ائتلاف حاکم 3. پیدایش گرو‌ه‌های مخالف؛ 4. انتخاب راهبرد مواجهه و تحقق خط‌‌مشی. همچنین، بر اساس این چارچوب 5 طبقه بازیگران و ائتلاف‌ها، باورها و مسائل، شرایط و رویدادهای بیرونی و درونی، تغییر و یادگیری مذاکره‌ای، منابع و محدودیت‌ها شناسایی شد.

Policy Analysis of the Health System Based on the Advocacy Coalition Frameworks (Case Study: Iran’s Health system Reform plan)

In this paper, Authors tried to explore and explain the development of the health system through the Advocacy Coalition framework as a framework for analyzing the process of change. The nature and objectives of the research required the use of a qualitative approach and two methods of library and interviewing. The snowball method has been used to select the interviewees and the thematic analysis method used to analyze the interview data. The results of semistructured interviews with 12 experts represent the results of the study indicate the ability of the Advocacy Coalition framework to explain the policy change process of the health system. The four steps in formulating of Iran’s Health system Reform plan are including: 1. National consensus; 2. Change in ruling coalition; 3. The emergence of opposition groups; 4. Selecting a strategy for confronting and implementing a policy.
