بررسی وضعیت تجویز پیشگیرانه آنتی‌بیوتیک‌ها در اعمال جراحی بخش جراحی عمومی بیمارستان شهید صدوقی یزد در سال 1396

مقدمه: عفونت محل جراحی از علل مهم ناتوانی و مرگ پس از عمل جراحی است و استفاده از آنتی‌بیوتیک پروفیلاکسی از روش‌های موثر پیشگیری از آن است. هدف از این مطالعه وضعیت تجویز پیشگیرانه آنتی‌بیوتیک‌ها در اعمال جراحی بخش جراحی عمومی بیمارستان شهید صدوقی یزد در سال 1396 بود.روش بررسی: در این مطالعه توصیفی مقطعی 600 بیمار که در سال 1396 در بخش جراحی عمومی بیمارستان شهید صدوقی یزد تحت عمل جراحی قرار گرفتند، بررسی شدند. اطلاعات مورد نیاز با استفاده از پرسش‌نامه از پرونده بیماران استخراج گردید. تجویز آنتی‌بیوتیک، نوع آن، راه تجویز و زمان شروع دارو با دستورالعمل کشوری و ashp (american society of health system pharmacistis) مطابقت داده شد تا همخوانی با دستورالعمل‌ها مشخص گردد. آنالیز داده‌ها با استفاده از نرم‌افزارspss version 16 انجام شد.نتایج: میانگین سن بیماران 19/24 ±35/02 به‌دست آمد. در مورد استفاده یا عدم استفاده از آنتی‌بیوتیک 85/2%، راه تجویز 85/2%، دوز دارو 64/7% همخوانی با دستورالعمل‌ها وجود داشت. زمان تجویز دارو در 72/3% موارد با دستورالعمل کشوری و در 72/8% موارد با ashp همخوان بود. بیشترین عدم همخوانی در مورد نوع آنتی‌بیوتیک مصرفی بود که در 48/8% با دستورالعمل کشوری و در 50/7% موارد با ashp همخوان بود. همخوانی کلی با ashp ودستورالعمل کشوری به‌ترتیب در در 50/7‌% و 48/8 %موارد وجود داشت. نتیجه‌گیری: در مطالعه اخیر بیشترین ناهمخوانی در مصرف آنتی‌بیوتیک پروفیلاکسی در اعمال جراحی در مورد نوع آنتی‌بیوتیک تجویزی بود.

Evaluation of the Status of Prophylactic Administration of Antibiotics in Surgery Department of Shahid Sadoughi Hospital of Yazd City in 2018

Introduction: surgical site infection is one of the most important causes of mortality and morbidity: use of the prophylactic antibiotic is one of the effective methods for its prevention, the present study aimed to investigate the status of prophylactic administration of antibiotics in Surgery Department of Shahid Sadoughi Hospital of Yazd City in 2018.Methods: this descriptive crosssectional study was performed on 600 patients from General Surgery Department of Shahid Sadoughi Hospital of Yazd City in 2018. Data were collected by questionnaire. Selection of the prophylactic antibiotic (pa), the dosage, time, type and the route of antibiotic administration were compared with ASHP ( American Society of Health System Pharmacistis) and Clinical Medicine Strategy of Using Preventive Antibiotics in Surgeries (Iranian guideline). Data were analyzed by SPSS version 16 software.Results: Mean age of the patients was 35.02 ±19.12 years. Regarding the use or nonuse of antibiotics 85.2%, the route of administration 85.2%, the dose of the drug 64.7% were in accordance with the instructions. The time of drug prescription was in accordance with the national instructions in 72.3% of cases and in accordance with ASHP in 72.8% of cases. The most inconsistency with ASHP and Iranian guideline was seen in the type of the antibiotic that was in accordance with ASHP and Iranian guideline in 50.7% and 48.8%, respectively. The overall appropriateness of PA was found in 50.7% and 48.8% cases according to ASHP and Iranian guideline, respectively. Conclusion: In the recent study, the most inconsistency with ASHP and Iranian guideline was seen in the type of prescription antibiotic.
