چکیده
|
مقدمه: لنفوم بدخیم به دو زیر گروه هوچکین و غیرهوچکین تقسیم میشود و 40% آنها در مناطقی غیر از گره لنفاوی بهوجود میآیند که شایعترین محل آن دستگاه گوارش است. بر اساس مطالعات، در سالهای اخیر شیوع لنفومهای غیرهوچکینی نسبت به قبل افزایش داشته است. لذا ما در این مطالعه بر آن شدیم تا لنفومهای دستگاه گوارش و میزان بقای بیماران مبتلا به آنها را بر اساس خصوصیات کلینیکوپاتولوژیکال بررسی کنیم.روش بررسی: مطالعه حاضر از نوع تحلیلیمقطعی بود. جامعه مورد بررسی، تعداد 97 بیمار مبتلا به انواع لنفوم دستگاه گوارش، مراجعه کننده به سه بیمارستان شهر یزد، طی سالهای 1395-1390 بودند و نمونهگیری به صورت سرشماری بود. با مراجعه به بخش پاتولوژی بیمارستانها،تمام موارد مربوط به لنفومهای دستگاه گوارش استخراج و اطلاعات مورد نظر (سن، جنس، سال تشخیص بیماری، نوع لنفوم، محل لنفوم، نوع درمان و میزان بقا) با استفاده از چکلیستی که از قبل تهیه شده بود، ثبت گردید. سپس اطلاعات بهدست آمده با نرمافزار spss inc.,chicago, il; version 18 تحت آنالیز آماری قرار گرفت و با استفاده از نمودارهای کاپلانمایر و آزمون لگرنک به بررسی بقا بیماران پرداخته شد.نتایج: نتایج نشان دادکه از 97 بیمار، 69 نفر (71.1%) مرد و 28 نفر (28.9%) زن بودند. میانگین سنی بیماران مورد بررسی، 21.5 ±49.28 بود. شایعترین محل لنفوم، معده (45.4%)، شایعترین شیوه درمان، جراحی+کموتراپی (53.6%) و شایعترین نوع لنفوم، لنفوم سلولb (50%) بوده است. میانگین مدت بقا بیماران، 48 ماه و بین میانگین مدت بقا برحسب نوع درمان رابطه آماری معناداری یافت شد (0.000=p).نتیجهگیری: با توجه به نتایج، میتوان نتیجهگیری کرد که درمان مناسب برای بیماران مبتلا به لنفومهای دستگاه گوارش، درمان توام جراحی+کموتراپی است.

Abstract
|
Introduction: Malignant lymphoma is divided into two subgroups of Hodgkin's and nonHodgkin's, and 40% of them occur in areas other than the lymph node, the most common being the digestive tract. According to studies, in recent years the prevalence of nonHodgkin's lymphomas has increased over the past. Therefore, in this study, we investigated the gastrointestinal lymphoma and the survival rate of the patients with this disease based on clinicopathologic characteristics.Methods: This study was an analyticcross sectional study. The study population included 97 patients with various types of gastrointestinal lymphoma referring to 3 hospitals in Yazd during 20112011. Sampling was done by census. Referring to pathology department of hospitals, all cases related to gastrointestinal tract lymphoma extracted and the information (age, sex, year of diagnosis, type of lymphoma, location of lymphoma, treatment method and survival rate) using the checklist was recorded. The data were analyzed by SPSS18 software and patients' survival was assessed using KaplanMeier curves and LogRank test.Results: The results of the study showed that out of 97 patients, 69 patients (71.1%) were male and 28 (28.9%) were female. The mean age of the patients was 49.28 ±21.5. The most common site of lymphoma was stomach (45.4%), the most common type of treatment, surgical+ chemotherapy (53.6%), and the most common type of lymphoma, B cell lymphoma (50%). The mean survival time was 48 months and there was a significant relationship between mean survival time according to type of treatment.Conclusion: According to the results, it can be concluded that the appropriate treatment for patients with gastrointestinal lymphomas is surgical+chemotherapy combination therapy.
