بررسی نتایج آزاد‌سازی به روش آرتروسکوپی در محدودیت حرکت زانو

مقدمه: محدودیت حرکت زانو یکی از عوارض نسبتاً شایع به‌دنبال تروما و جراحی می‌باشد. با تشخیص زودرس و انتخاب درمان مناسب، بازگرداندن حرکت زانو و بهبود عملکرد خیلی از بیماران مورد انتظار می‌باشد. در هر حال، محدودیت حرکات زانو، ناخوشایند بوده و باید در اسرع وقت اصلاح شود. در این مطالعه ما بر آن شدیم تا نتایج آزاد‌سازی به روش آرتروسکوپی در محدودیت حرکت زانو را بررسی کنیم.روش بررسی: نوع مطالعه توصیفی‌ مقطعی بود. 14 بیمار مراجعه کننده به بیمارستان شهید صدوقی یزد از سال 1393 تا 1395 با تشخیص محدودیت حرکت زانو (محدودیت فلکسیون یا اکستانسیون بیش از 10 درجه) انتخاب شدند. بیماران تحت آزاد‌سازی آرتروسکوپی قرار گرفتند و تا 6 ماه پیگیری شدند. lysholm knee score، درجه فلکسیون و اکستانسیون و میزان رضایت‌مندی بیماران بررسی شد. نتایج حاصل، با نرم‌افزار spss inc., chicago, il; version 18 و آزمون‌های آماری paired ttest و spearman correlation مورد تجزیه و تحلیل قرار گرفت.نتایج: میانگین سنی بیماران 10.99 ± 28.92 بود. شایع‌ترین نوع تروما در بیماران، شکستگی دیستال فمور با 46.7 % بود. در مورد فراوانی رضایت بیماران از آرتروسکوپی،50% (7نفر) رضایت متوسط، 28.6% (4 نفر) رضایت زیاد و 21.4% (3 نفر) رضایت کم داشتند. اختلاف میانگین lysholm knee score، بهبودی درجه اکستانسیون، بهبودی درجه فلکسیون و نمره درد، قبل و بعد از آرتروسکوپی به‌ترتیب 14.11 ±26.21، ±26.13 64.78 ،2.92 ±3.57و 1.65 ±4.13 بود. بین اختلاف میانگین فاکتورهای مذکور، قبل و بعد از آرتروسکوپی تفاوت آماری معناداری یافت شد.نتیجه‌گیری: با توجه به نتایج به‌دست آمده می‌توان نتیجه‌گیری کرد که آزاد‌سازی آرتروسکوپیک یک روش موثر و کم‌عارضه در درمان محدودیت حرکات زانو می‌باشد.

Evaluation of the Results of Arthroscopic Release in Limits of Movement in Knees

Introduction: Limits of movement in knees is one of the most common complications following trauma and surgery. With early diagnosis and appropriate treatment choices, knee replacement and performance improvement are expected in many patients. However, limits of movement in knees is unpleasant and should be corrected as soon as possible. In this study, we decided to evaluate the results of arthroscopic release in motion limits of knees.Methods: This was a descriptive crosssectional study.14 patients who referred to the Shahid Sadoughi Hospital of Yazd from 2014 to 2016 and diagnosed with knee limitation (flexion or extension restriction of more than 10 degrees) were selected.Patients were undergone arthroscopic release and followedup for 6 months. Lysholm knee score, degrees of flexion and extension, and patient satisfaction were evaluated. The results were analyzed by SPSS18 and statistical tests Paired Ttest as well as Spearman correlation.Results: The mean age of the patients was 28.92 ±10.99 years.The most common type of trauma in the patients was femoral distal fracture (46.7%). The frequency of the patients' satisfaction with arthroscopy was as followed: 50% (7 patients), had moderate satisfaction, 28.6% (4 ones), high satisfaction, and 21.4% (3 ones) had low satisfaction.The mean difference in Lysholm Knee Score, improvement in degree of extension, improvement in degree of flexion and pain score before and after arthroscopy were 26.21 ±14.11,64.78 ±26.13,3.57 ±2.92 and 4.13 ±1.65,respectively.There was a significant difference between the mean values of these factors before and after arthroscopy.Conclusion: According to the results obtained, we can conclude that arthroscopic release is an effective and lowcomplication method in motion limits of knees.
