بررسی شیوع بولتون دیسکرپانسی و مال اکلوژن های مرتبط با آن در بیماران مراجعه کننده به بخش تخصصی ارتودنسی دانشکده دندانپزشکی کرمان و مطب‌های خصوصی بین سال های 1394-1391

مقدمه: به منظور برقراری اکلوژن مناسب با اورجت و اوربایت صحیح و هماهنگی قوس ها در مراحل پایانی درمان ارتودنسی نیاز به تناسبی ویژه میان اندازه دندان های بالا و پایین است. در این راستا به هنگام تشخیص، طرح ریزی درمان و تعیین پیش آگهی درمان ارتودنسی باید آنالیز بولتون مورد توجه قرار گیرد. هدف از این پژوهش تعیین شیوع بولتون دیسکرپانسی در بیماران مراجعه کننده به دانشکده دندان پزشکی کرمان و کلینیک های خصوصی و تعیین اثرات جنس و نهفتگی یک طرفه دندان کانین بالا بر روی نسبت های بولتون می باشد.روش بررسی: این مطالعه مقطعی بر روی کست های پیش از درمان311 بیمار مراجعه کننده به دانشکده دندانپزشکی کرمان و مطب های خصوصی بین سال های 1394-1391 انجام شد. بزرگ ترین بعد مزیودیستالی دندان های مورد نظر با کولیس دیجیتال با تقریب 0/01 میلی متر اندازه گیری شد. سپس، نسبت های بولتون محاسبه گردید. جهت آنالیز داده ها از آزمون t-test و مجذور کای با کمک نرم افزار spss 18 استفاده شد و p<0/05 به عنوان سطح معنی داری در نظر گرفته شد.نتایج: میانگین بولتون قدامی در جمعیت مطالعه حاضر بدون در نظر گرفتن نوع مال اکلوژن وجنسیت 78/49% و بولتون کلی 91/73 % به دست آمد. شیوع بولتون قدامی 41/5 % و بولتون کلی 25/7 % بود. بولتون قدامی و کلی در زنان بیشتر از مردان بود. ارتباط معنی داری بین نهفتگی یک طرفه دندان کانین بالا و بولتون دیسکرپانسی قدامی و کلی وجود نداشت.نتیجه گیری: این تحقیق نشان دهنده شیوع قابل توجه دیسکرپانسی سایز دندانی در بیماران می باشد. با توجه به اختلالات اندازه دندان ها و تاثیر آن بر اکلوژن و روابط دندانی، توصیه می شود آنالیز بولتون قبل از شروع درمان ارتودنسی در مورد بیماران انجام شود.

The prevalence of Bolton discrepancy and associated malocclusion in patients attending the orthodontics department of Kerman Dental School and private clinics: 2012 2015

Introdution: For the proper occlusal interdigitation or coordination of arches in the finishing stage of orthodontic treatment with proper overjet and overbite, there must be a proper mesiodistal tooth size ratio between maxillary and mandibular teeth. The Bolton analysis should be taken into consideration while diagnosing, treatment planning and predicting prognosis in clinical orthodontics. The purpose of this study was to evaluate the prevalence of Bolton discrepancy in the patients attending to Orthodontics Department of Kerman Dental School and private clinics to determine the effects of gender and unilateral maxillary canine impaction on Bolton ratio.Methods: This cross sectional study was performed on 311 pretreatment orthodontic casts of patients attending to Orthodontics Department of Kerman Dental School and private clinics between 2012 2015. The greatest mesiodistal dimensions of teeth were measured with digital caliper with accuracy of 0.01 mm. Independent ttest and chi square were applied to analyze the data by SPSS 18 software and values were considered to be significant when P<0.05.Results: The mean anterior and overall Bolton rsquo;s ratios were 78.49% and 91.73%, respectively without considering malocclusion and gender. The most prevalent anterior and overall Bolton ratios were 41.5% and 25.7%, respectively. The anterior and overall Bolton ratios for female patients were greater than male patients. There were no significant relationship between canine impaction and Bolton rsquo;s ratios.Conclusion This study indicated a noticeable prevalence of tooth size discrepancies in the patients. Regarding tooth size discrepancies and their effects on occlusion and dental relations, it is suggested that Bolton rsquo;s tooth size analysis must be performed before starting the orthodontic treatment
