چکیده
|
زمینه: حفظ فشار کاف در محدوده مناسب (25 تا 30 سانتی متر آب) در پیشگیری از بروز عوارض مربوطه نقش مهمی دارد.هدف: مطالعه به منظور تعیین اثر تغییر وضعیت بدن بر میزان فشار کاف لوله داخل نای در بیماران ضربه به سر تحت تهویه مکانیکی بستری در بخش های مراقبت ویژه انجام شد.مواد و روش ها: این مطالعه نیمه تجربی بر روی 30 بیمار ضربه به سر دارای لوله نای تحت تهویه مکانیکی، بستری در بخش های مراقبت ویژه بیمارستان های شهید رجایی و رازی قزوین در سال 1393 به روش نمونه گیری در دسترس انجام شد. میانگین فشار کاف لوله نای برحسب سانتی متر آب در شش وضعیت مختلف بدن در فواصل زمانی دو ساعته با استفاده از مانومتر استاندارد، بلافاصله قبل، بلافاصله بعد، 15 دقیقه و 2 ساعت بعد از تغییر وضعیت بیمار اندازه گیری شد. داده ها با آزمون های آماری کای دو، تی مستقل و بن فرنی تحلیل شدند.یافته ها: میانگین فشار کاف لوله نای در تغییر وضعیت خوابیده به پهلوی چپ و راست با زاویه 30 درجه و سر در راستای بدن به ترتیب 0/68±31/5و 0/68±34/1سانتی متر آب و همچنین خوابیده به پهلوی چپ و راست با زاویه 45 درجه و سر در راستای بدن در بازه زمانی 15 دقیقه و 2 ساعت بعد از تغییر وضعیت بیش تر از محدوده طبیعی بود (به ترتیب 0/92±31/9 و 1/5±30/4سانتی متر آب). در همه موارد تفاوت میانگین فشارها از لحاظ آماری معنی دار بود (0/001>p).نتیجه گیری: با توجه به تغییر غیرطبیعی فشار کاف لوله نای به دنبال تغییر وضعیت بدن، لازم است پس از تغییر وضعیت بیمار، فشار کاف اندازه گیری و در صورت لزوم تصحیح شود.

Abstract
|
Background: Maintaining endotracheal tube cuff pressure in the normal range of 2530 cmH2o plays an important role in the prevention of related complications.Objective: The present study aimed to determine the effect of body change position on level of endotracheal tube cuff pressure in patients with head trauma under mechanical ventilation admitted in ICUs.Methods: This quasiexperimental study was implemented on 30 patients with head injury under mechanical ventilation who admitted in ICUs in 2014. Available method was used for sampling. The mean of endotracheal cuff pressure was measured in 6 different body positions in the twohour intervals using standard manometer, immediately before, immediately after, 15 minutes and 2 hours after the change positions. Data was analyzed using Chisquare, t and Bonferroni tests.Findings: The mean of endotracheal cuff pressure in the left and right lateral position with an angle of 30 degrees and head to the body (Respectively 31.5±0.68 and 34.1±0.68 cmH2o) and also the left and right lateral and a 45degree angle to the body at the end of the period of 15 minutes and 2 hours after change positions was higher than the normal range (Respectively 31.9±0.92 and 30.4±1.5 cmH2o). In all cases, the mean of pressure differences were statistically significant (plt;0.001)Conclusion: According to the unnatural changes in endotracheal tube cuff pressure due to body change positions, it is suggested that after changing position of the patient, measure the cuff pressure and correct it if itamp;rsquo;s necessary.
