تغییرات آناتومیکی عصب فرنیک نسبت به شریان توراسیک داخلی

مقدمه: عصب فرنیک (phrenic nerve) عصبی حسی و حرکتی است و به دلیل عصب دهی عضله دیافراگم ازلحاظ آناتومی و بالینی اهمیت دارد. هدف از این گزارش بیان تغییر آناتومیکی از یک مورد نادر در ارتباط با عصب فرنیک و شریان توراسیک داخلی (internal thoracic artery) است.مواد و روش ها: در طی تشریح معمولی جسد مردی 53 ساله در ناحیه ریشه گردن مشخص شد که قسمت ابتدایی شریان توراسیک داخلی توسط حلقه‌ای از عصب فرنیک راست در برگرفته ‌شده و سپس به سمت پایین ادامه مسیر داده است.یافته ها: تغییرات آناتومیکی در مورد عصب فرنیک و شریان توراسیک داخلی همواره از اهمیت بالینی ویژه‌ای برخوردار است و اگر در جراحی‌های منطقه‌ای مورد توجه قرار نگیرد می‌تواند منجر به عوارض بالقوه تهدید کننده زندگی شود.بحث و نتیجه گیری: بررسی این مورد نشان داد که عصب فرنیک و شریان توراسیک داخلی می‌توانند نسبت به یکدیگر موقعیت‌های متفاوتی را نشان دهند. به نظر می‌رسد چون بخش اول و دوم دیواره‌ی شریان بافت الاستیکی دارد می‌تواند با تحت تاثیر قرار دادن عصب فرنیک، عصب دهی عضله دیافراگم را مختل کند.

background: the phrenic nerve is a sensory and motor nerve and is important from an anatomical and clinical point of view due to the innervation of the diaphragm muscle. the present report aimed to to describe the anatomical variation of a rare case related to the phrenic nerve and the internal thoracic artery.materials and methods: during the routine autopsy of a 53-year-old man, it was found that the initial part of the internal thoracic artery was covered by a ring of the right phrenic nerve and then continued downward.results: anatomical changes in the phrenic nerve and internal thoracic artery are always of special clinical importance, and if not considered in regional surgeries, they can lead to potentially life-threatening complications.conclusion: the investigation of this case showed that the phrenic nerve and the internal thoracic artery can show different positions relative to each other. it seems that since the first and second parts of the artery wall have elastic tissues, it can disrupt the innervation of the diaphragm muscle by affecting the phrenic nerve.
